| Objective: Differences of diagnostic efficacy between 1.5T and 3.0T MRI were evaluated in the patients with wrist soft tissue injury to provide a more objective evidence in choosing different MRI for clinical diagnosis.Methods: 45 patients with wrist pain as the research objects in He Bei province CangZhou hospital of TCM and WM were involved in the study group druing May 2016 to November 2017.After X-ray scanning,the distal radius fracture was excluded from the group.The patients with wrist triangular fibrocartilage complex(TFCC)injure or(and)scapholunate interosseous ligament(SLIL)injure or(and)lunotriquetral interosseous ligament(LTIL)or without a obvious damage were preliminary diagnosed by imaging specialist after further examination of 1.5T MRI and 3.0T MRI.Meanwhile all of the patients were undergone wrist arthroscopy diagnostic surgery in the department of hand microsurgery.The 1.5T MRI and 3.0T MRI results were compared with the wrist arthroscopic diagnostic results,taking the wrist arthroscopic results as a “gold”standard to evaluate the differences of diagnostic efficacy between 1.5T MRI and 3.0T MRI in the wrist soft tissue injury.Results:1.1.5T magnetic MRI results: wrist TFCC injury(24 cases),SLIL injury(7 cases),TFCC injury combined with tears of the LTIL(3 cases)and no injury(11 cases);3.0T MRI diagnostic results: wrist TFCC injury(23 cases),SLIL injury(13 cases),TFCC injury combined with tears of the LTIL(5 cases)and no injury(4 cases);Wrist arthroscopic diagnostic results: wrist TFCC injury(20 cases),SLIL injury(15 cases),TFCC injury combined with tears of the LTIL(9 cases)and no injury(1 cases).2.Research shows that the diagnosis efficiency of the 1.5T MRI in wristinjury of TFCC can reach to 93.10% and 96.55% in 3.0T MRI,and there is no correlation between the diagnosis efficiency of MRI and the injury type and classification of the TFCC(P > 0.05),there is correlation in the diagnosis of wrist injury of TFCC between 1.5T MRI and 3.0T MRI and wrist arthroscopy(P< 0.05);However,there is no significant difference in the diagnosis rate of SLIL injury and LTIL injury in the patients with 1.5T MRI and 3.0T MRI(P> 0.05).Conclusion: Although both 1.5T and 3.0T MRI can accurately show the wrist TFCC injury,since the sensitivity of 100% can not be reached at the moment,the MRI cannot replace the wrist arthroscopy.Although there is no obvious statistical difference between 1.5T MRI and 3.0T MRI in the diagnosis results of SLIL injury,a higher diagnosis efficiency can be found in3.0T MRI in a small sample experiment of wrist SLIL injure,therefore we prefer to recommend 3.0T MRI in the diagnosis of SLIL injury in the clinical;However,in the diagnosis of LTIL injury,both 1.5T and 3.0T MRI have a low diagnostic sensitivity and specificity.So we suggest applying wrist MRA examination or wrist arthroscopy for an accurate diagnosis in wrist LTIL injury. |
