| Objective:to investigate the risk factors and the related predictive indexes in patients of liver cirrhosis caused by different inducements which were accompanied with portal vein thrombosis.Method:Data collection including 636 cirrhotic inpatients who had done the computed tomographic angiography of portal vein system and who had been admitted to the Second Affiliated Hospital of Chongqing Medical University from January 2010 to December 2015.132 of them with portal vein thrombosis were classified as PVT group and 504 of them without portal vein thrombosis were classified as non-PVT group.The two groups were test by an univariate analysis and multiple logistic regression analysis in attempt to identify the risk factors and predictive indexes.Result:univariate analysis showed that Child-pugh score、portal vein velocity、spleen vein width、the width of the gastric-renal shunt vein 、the patency of the collateral circulations 、 pathogenesis of cirrhosis 、splenectomy achieved statistical significance between the twogroups(P<0.05);while there are no significant difference in age、sex、hepatic encephalopathy、ascites、albumin、TBil、PT、INR、D-dimer、PCT、WBC、CRP、 indicators of thrombelastogram、various indicators of color Doppler ultrasound(portal vein width、spleen vein velocity、the width and velocity of superior mesenteric vein)and the portal vein velocity below15cm/s between the two groups(p>0.05).The multiple multiple logistic regression showed that the Child-pugh score 、 portal vein velocity and splenectomy can be the independent risk factors for the formation of PVT.Conclusion: the Child-pugh score rising、the portal vein velocity decline and splenectomy are the independent risk factors for the PVT,but we find that the portal vein velocity below 15cm/s can not predict the PVT for the present,which is not consistent with some reports nowadays.So we should supervise and adjust those indicators in case of the occurance of PVT and so as to dispose and interfere clinically in time.At the same time,we hope it can not only reduce the motality,but also improve the life quality. |
