Analysis The Risk Factors And Patterns Of NMIBC Recurrence After TURBt

Objective:To retrospectively analyze factors of recurrence of NMIBC after TURBT,clear affecting the risk factors of recurrence and the site of recurrence,to provide effective reference for high-risk patients regularly follow-up.Methods:Retrospective analysis of the April 2013-2015 year in April in the Affiliated Hospital of Qingdao University underwent transurethral resection of bladder tumor(TURBT)treatment of 391 patients,the pathological stage Ta,T1,and have complete clinical medical data,the recurrence of bladder cancer after follow-up,location and time,the recurrence of production the database,using SPSS22.0 statistical analysis,,Kaplan-Meier was used to plot the survival time curve without recurrence,the comparison between the two groups using Log-Rank,finally multivariate analysis using Cox proportional hazard model.Results:To the follow-up time(December 31,2017),according to the 2009 edition of UICC staging,tumor recurrence in patients with first stage non invasive papillary carcinoma(Ta)accounted for 20%,invaded the lamina propria of the tumor(T1)accounted for 80%,The results showed that 65 of 391 patients had recurrence,the total recurrence rate was 16.62%,the average recurrence free survival time was 37.76 months,including 13 cases of recurrence in site(20%),36 cases of ectopic recurrence(55.4%),multiple tumor recurrence in 16 cases(24.6%),By the single factor analysis,according to the results of patients with tumor in patients with single and multiple average relapse-free survival time was 59.006 ±0.929 、43.64±1.659(P = 0.000),bladder tumor history average recurrence relapse-free survival time was 56.354±0.986 、49.105±3.380(P = 0.009),histological grade low potential of malignant urothelial tumor relapse-free survival time is 46.294 ±1.655,low level urothelial carcinoma is 58.619 ±1.057,the high level of urothelial carcinoma is 50.233 ±1.773(P = 0.000),patients with pathological staging Ta relapse-free survival time is 61.316± 0.731,T1 is 39.642 ±1.743(P = 0.000),without squamous differentiation Cases with squamous differentiation average relapse-free survival time was 56.374 ±0.946 、46.320 ±1.655,(P = 0.033),Multivariate analysis of Cox proportional risk model showed that tumor staging(P=0.000)、 multiple tumor(P=0.002)were the independent influencing factor for recurrence after TURBT.Conclusion:The conclusion is still high in non muscle invasive bladder cancer after transurethral resection of the recurrence rate,the main patterns of recurrence for ectopic recurrence,suggesting that bladder cancer incidence may be multi-center.Multiple tumor,previous recurrence,histological grade,tumor stage,differentiation of squamous cell carcinoma are associated with factors influencing postoperative recurrence.By multivariate analysis,tumor staging and multiple tumor are independent factors.
