ObjectiveBy analyzing the clinical symptoms,imaging and pathological features of common salivary gland benign and malignant tumors,and studying the diagnostic coincidence rate of preoperative adjuvant examination,to provide a rapid,simple,effective and low-cost diagnostic method for tumor-like tumors before operation.MethodsTo collect the common cases of salivary gland tumors in the Department of Stomatology,first affiliated Hospital,School of Medicine,Shihezi University,from January 2000 to September 2017,including pleomorphic adenoma,adenoma,basal cell adenoma,mucoepidermoid carcinoma,adenoid cystic carcinoma,and malignant mixed tumor.By collecting the clinical data,auxiliary examination,pathological data,analyze the sex,histological classification,site composition and clinical examination of salivary gland tumor and performing the gold standard paraffin section examination of salivary gland tumor,to analysis the coincidence rate of preoperative(B-ultrasound,fine needle aspiration cytology)and intraoperative diagnosis(frozen section microscopic examination).The data are statistically processed,SPSS19.0software is used to analyze,The result of the rate is described as a percentage.chi-square criterion or paired chi-square criterion were used in the comparison between groups,P<0.05 was statistically significant.Result(1)A total of 274 cases of salivary gland tumors were collected,including 112 pleomorphic adenomas(40.9%),102 adenomas(37.2%)and 13 basal cell adenomas(4.7%).The most common malignant tumors were mucoepidermoid carcinoma(17 cases)and adenoid cystic carcinoma(17 cases).The ratio of men to women with salivary gland tumors was 1.95:1(181/93),and the ratio of men to women with adenolymphoma was 10.33:1(93/9).The average onset age of benign tumors was 53.64 years,the mean age of malignancy was 53.72 years old.(2)The statistical results of body examination showed that there were differences in texture,boundary,tenderness,mobility and facial paralysis between benign and malignant tumors,but no difference in lymph node enlargement.(3)Compared with paraffin section,the overall coincidence rate of B-ultrasonography and paraffin section was 83.2%,among which the diagnostic coincidence rate of benign tumor and malignant tumor was85.2%and 73.3%respectively.(4)The coincidence rate of qualitative diagnosis between fine needle aspiration cytology and paraffin section was 87.0%,in which the benign tumor was 88.6%and the malignant tumor was 79.2%.The total coincidence rate of diagnosis was 63.8%,with benign tumor 64%and malignant tumor 62.5%.(5)The coincidence rate of qualitative diagnosis between frozen section and paraffin section during operation was 94.9%,97.4%for benign tumor and 83.0%for malignant tumor.The diagnostic coincidence rate was 88.0%,including 92.5%for benign tumor and 66th for malignant tumor.(6)There was no significant difference between FNAC and B-ultrasound in qualitative diagnosis coincidence rate(χ~2=3.56,P>0.05).The difference between frozen section and B-ultrasound was statistically significant(χ~2=32.09,P<0.05).The difference between intraoperative frozen section examination and fine needle aspiration cytology was statistically significant(χ~2=4.35,P<0.05).ConclusionThe coincidence rate of intraoperative frozen section in diagnosis of salivary gland tumor was higher than that in fine needle aspiration cytology and B ultrasonic examination,however,fine needle aspiration cytology and B-ultrasound play an important role in preoperative diagnosis and treatment planning.
