Comparison Of Clinical Curative Effect Of Total Hip Arthroplasty Reserving Articular Capsule In The Treatment Of Femoral Neck Fracture

Objective:Retrospective analysis of total hip arthroplasty(THA total,hip arthroplasty)in conventional capsule resection and repair of the joint capsule treatment of two kinds of joint capsule treatment of femoral neck fractures in the elderly(FNF,femoral neck fracture(Garden IV)the clinical curative effect of the postoperative radiographic results,Harris score,the rate of hip dislocation.Stand up,walk time)and safety(the amount of bleeding,intraoperative drainage,operation time,hospitalization time difference),and explore its significanceMethod:The Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from June 2015 to June 2016 for the diagnosis of patients admitted patients with type Garden IV and femoral neck fracture,retrospective analysis of the diagnosis,inclusion,exclusion criteria were 40 cases,including 20 cases of patients with resection of the joint capsule,and 20 cases with disease repair.Patients in two groups were recorded before operation.Intraoperative and postoperative early stage data,including the index of curative effect(postoperative hip dislocation rate,Harris score,time to get up to stand,walk time)and safety indicators(blood loss,intraoperative drainage,operation time,hospitalization time),the The results were statistically analyzed.There was no significant difference between the two groups in mean age,sex,weight and follow-up time(p>O.05)Results:1.The general data of the two groups of patients(age,weight,gender),P value greater than 0.05,the difference was not statistically significant 2.Two groups of patients with the Harris score,the difference was not significant(P> 0.05);after January when reserving articular capsule of patients with Harris score was 58.90 + 2.63,65 + capsule resection group was 2.75,significant difference(P< 0.05);after 3 months after the Harris score of the two groups were 84.65 + 2.56 and 84.55 + 2.24,the difference was not significant(P> 0.05),6 months after surgery,after two groups of patients with Harris The scores were 90.30 + /-2.18 and + 90.40 + 1.79,respectively.The difference was not significant(P> = 0.05)3.Comparison of two groups of patients with operation time,P < 0.05,showed a statistically significant difference in the operation time of the two groups of patients,resection of joint surgery time capsule group than reserving articular capsule group short;two groups of patients with blood loss,P value is less than 0.05,indicating that the two groups of patients in the amount of bleeding on the difference was statistically significance of capsule resection group hand bleeding than reserving articular capsule group;two groups of patients in hospitalization time,P < 0.05,showed that two groups of patients with postoperative hospitalization time difference was statistically significant.4.The two groups of flow rate,the time of abandoning the walking,P value of >0.05,the difference was not statistically significant 5.The experimental group did not occur early dislocation after operation(1 months after operation);the control group of 1 cases of early dislocation(1 months after operation(5%),two;dislocation rate)group after 1 months later there was no dislocation,delayed dislocation was not statistically significant.Conclusion: In the modified Gibson approach for total hip arthroplasty,the posterior capsule repair can be carried out in accordance with the method of repairing;reserving articular capsule THA with surgical trauma and less bleeding,shorten hospitalization time,early Harris score satisfied the advantages of proper patient selection has better clinical curative effect to equipment.
