Objectives To survey the prevalence of postpartum depression in high-risk pregnant women and their spouses;To analyze the effects of different demographic characteristics and obstetric factors on postpartum depression of high-risk pregnant women and their spouses;To explore the correlation between postpartum depression and parenting sense of competence and social support in high-risk pregnant women and their spouses,providing scientific basis for medical treatment and psychological nursing.Methods The method for convenient sampling was adopted in October 2016 to May 2017 by the Critical Maternal Care Center in a Grade III A hospital in Guangzhou city,in which 196 paired high-risk pregnant women and their spouses were eligible.High-risk pregnant women and their spouses were participated in the investigation in a week after delivery with the Edinburgh Postnatal Depression Scale,general information questionnaire,Chinese version of the Parenting Sense of Competence Scale,Social Support Rating Scale,and an analysis on social characteristics and obstetrical factors were done.All the data obtained was double entered using Epidata 3.1 software,and SPSS 22.0 statistical software carried out a statistical analysis,which was found to be statistical significant in P<0.05.Results Part Ⅰ 1.This study included 196 high-risk pregnant women,with postpartum depression were 58 cases,the incidence of postpartum depression was 29.6%;Postpartum depression in high-risk pregnant women was statistically significant in the general information about the living conditions,marriage,relationship in-laws,life events,and whether pregnancy was planned(P<0.05);Postpartum depression in high-risk pregnant women was statistically significant in the obstetrical factors about fetal distress,delivery mode,neonatal feeding(P<0.05),the associated factors of postpartum depression of high-risk pregnant women was life events,cesarean section,spouse with postpartum depression,OR and 95%CI values were 2.875(1.424~5.804),6.163(1.669~22.757),2.806(1.184~6.650).2.Postpartum depression in high-risk pregnant women is negatively correlated with the parenting sense of competence(r=-0.380,P<0.001)and social support(r=-0.187,P=0.009).2.Postpartum depression in high-risk pregnant women is negatively correlated with the parenting sense of competence(r=-0.380,P<0.001)and social support(r=-0.187,P=0.009).Part Ⅱ 1.This study included 196 spouses,with postpartum depression were 37 cases,the incidence of postpartum depression was 18.9%;Postpartum depression in spouses was statistically significant in the general information about the average monthly income,living conditions,marriage,life events(P<0.05);Postpartum depression in spouses was statistically significant in the obstetrical factors about postpartum hemorrhage,the placenta abnormality,delivery gestational age,neonatal body mass(P<0.05);the associated factors of postpartum depression of spouses was living conditions,the placenta abnormality,delivery gestational age,high-risk pregnant women with postpartum depression,parenting sense of satisfaction,OR and 95%CI values were 8.789(1.370~56.394),9.065(1.966~41.796),4.916(1.688~14.315),2.811(1.130~6.995),0.842(0.788~0.899).2.Postpartum depression in spouses is negatively correlated with the parenting sense of the parenting competence(r=-0.533,P<0.001),and social support(r=-0.193,P=0.007).Part Ⅲ Postpartum depression scores in high-risk pregnant women and their spouses were positively correlated(r=0.321,P<0.001).Conclusion 1.When postpartum depression occurs in high-risk pregnant women,their total score of parenting sense of competence was at a low level,objective support and utilization of support still needs to be improved;the influence factors of postpartum depression in high-risk pregnant women was life events,cesarean section,spouses with postpartum depression.2.When postpartum depression occurs in spouses,their total score of parenting sense of competence was at a low level,objective support and utilization of support still needs to be improved;the influence factors of postpartum depression in spouses was living conditions,the placenta abnormality,preterm birth,high-risk pregnant women with postpartum depression,the parenting sense of satisfaction was at a low level.3.Postpartum depression is a independent risk factor for high-risk pregnant women and their spouses.
