Correlation Study Between The Nerve Root Sedimentation And Expectant Treatment Of The Degenerative Lumbar Spinal Stenosis

Objective: Using the Bolender classification method and NRT image,to classificate the patients with Conservative treatment of degenerative lumbar spinal stenosis with Chinese and Western Medicine,By observing the patients before and after treatment with VAS and ODI score and the improvement of efficacy,to explore the effect of NRT on the conservative treatment of patients with LSS,to explore whether the treatment strategy for the development of reference.To explore whether it can be used for reference in the formulation of treatment strategies.Method:In this study,we selected 60 patients who were diagnosed as central DLSS in the Department of orthopedics of Affiliated Hospital of Chengdu University of TCM /Sichuan Provincial Hospital of traditional Chinese Medicine from January2015 to June 2016,all patients underwent routine lumbar MRI.According to the Boleder classification [42] definition,a retrospective study was performed on 60 patients who met the criteria for inclusion criteria and the dural sac area was80-120mm2,There were 29 males,aged 50~72 years,with an average age of about years old,a female of 31 cases and a female patient aged 51~74 years,with an average age of about 63.6 years,were divided into positive group and negative group according to NRT imaging method;A total of 35 patients in the positive group and a total of 25 patients in the negative group were retrospectively reviewed.1 By analyzing the correlation between the two imaging methods.2.By observing the system after conservative treatment of Chinese and Western medicine,applicating of “Chinese disease diagnosis and efficacy standards",VAS ODI,standard to evaluate the pain,function and daily life improvement of the patients at the time of admission and after treatment and 1months after treatment,and use Statistical analysis of the data,analysis of imaging and clinical evaluation of two groups of patients after treatment in all aspects of improvement,to explore the effect of NRT on the efficacy of conservative treatment of DLSS.Result:After statistical analysis,There was no statistical significance betweenthe two groups in gender,age of onset,clinical classification,and so on.In the statistical results,using person correlation analysis to analysis of VAS and ODI score of dural sac cross-sectional area(CSA)before and after treatment,The data showed no correlation between CSA and VAS score before treatment(OR=0.075)and ODI score before treatment(OR=0.032);At the same time that the average CSA nerve root sedimentation area syndrome positive group patients were:(96.77 + 11.107mm2),nerve root sedimentation average CSA area syndrome negative patients:(103.92 +9.587mm2),two groups of patients before treatment with significant difference CSA area(P < 0.05),with statistical significance,In the treatment,the VAS and ODI scores of two groups before and after treatment were compared and analyzed between the positive group and the nerve root subsidence negative group,There was no significant difference in VAS score and ODI score between the two groups before treatment,(P>0.05),In the treatment improvement,By combining traditional Chinese and Western medicine conservative treatment in all patients after treatment,VAS and ODI score were improved with different degree,VAS and ODI of the two groups after treatment were compared and analyzed,the two groups after treatment VAS score and ODI score had no obvious statistical difference(P < 0.05);there was no statistically significant difference,while the two group patients after the January VAS and ODI score improvement in comparison,there are significant differences between the two groups(P < 0.01).Conclusion:In the patients with CSA located in the interval of 80-120mm2,the incidence of positive nerve root subsidence was correlated with the area of CSA,and the smaller the CSA,the more likely to be positive for nerve root subsidence.At the same time it shows the nerve root sedimentation sign for the diagnosis of lumbar spinal stenosis,and does not reflect the advantages compared to other imaging methods.The patients with lumbar spinal canal stenosis,the patients with positive nerve root subsidence may be more serious than the other segments,but the long-term effect is similar to that of conservative treatment.Nerve root subsidence can provide clinical reference for the treatment of lumbar spinal stenosis.
