Objective:To explore the correlation between the detection index of ultrasonography and vascular endothelial growth factor(VEGF)and microvascular density(MVD),and to evaluate its clinical significance.To evaluate the clinical value of ultrasonography in qualitative and quantitative determination of benign and malignant thyroid nodules.Methods:The thyroid nodule was found in the department of ultrasonography of the first clinical medical college of shanxi medical university from January,2016 to April,2017.In addition,55 patients and 70 nodules were treated in this hospital.Contrast-enhanced ultrasound(CEUS)was performed before the operation.Sono Vue was used to observe the enhancement mode of thyroid nodule and the TIC curve analysis was performed.Immunohistochemistry was performed to detect VEGF,MVD,and the staining of MVD and VEGF,and the results were statistically analyzed.Results:(1)55 cases of 70 thyroid nodules were contrast-enhanced ultrasound(CEUS)check:including malignant thyroid nodules,a total of 41,thyroid papillary carcinoma(15/41)respectively,tiny papillary carcinoma(24/41),medullary carcinoma(1/41),anaplastic carcinoma(1/41);The benign nodules of the thyroid gland were 29,respectively,nodular goiter(18/29),adenoma(5/29),hashimoto's thyroiditis(5/29),and subacutegranulomatous thyroiditis(1/29).1 the two-dimensional ultrasound image characteristics of thyroid nodules and pathologic findings contrast two-dimensional ultrasound image characteristics of the boundary,shape,microcalcification was statistically significant(P < 0.05),while edge,aspect ratio,rear echo in this study had no statistical significance;In benign and malignant thyroid nodules,of a CDFI ultrasound image characteristics of malignant nodules with vascular edges(23/41)and non vascular(14/41),and benign nodules with mixed vascular(11/29)and non vascular(10/29).2 main ultrasound imaging performance for malignant thyroid nodule enhancement boundary was not clear or less(31/41),centripetal enhancement(35/41),the uneven distribution of intensity(31/41),benign nodules enhancement clear boundary(23/29),diffuse enhancement(20/29),uniform intensity distribution(18/29).The enhancement mode of thyroid nodules was mainly manifested as low enhancement(28/41).If low enhancement was used to diagnose malignant thyroid nodules,the sensitivity of CEUS was 68.29% and the specificity was 65.51%.The enhancement mode of benign thyroid nodules was mainly manifested as iso-enhancement(17/29).If the standard of benign thyroid nodules was enhanced,CEUS sensitivity was 68.29%,and the specificity was 53.65%.3 There was no statistically significant difference in the difference between the TTP,RT,the TTP,and the RT.(2)55 cases of 70 thyroid nodules were performed immunohistochemical detection:pathology of malignant thyroid nodules,a total of 41,thyroid papillary carcinoma(15/41)respectively,tiny papillary carcinoma(24/41),medullary carcinoma(1/41),anaplastic carcinoma(1/41);A total of 29 thyroid nodules were nodular goiter(18/29),adenoma(5/29),hashimoto's thyroiditis(5/29),and subacute granulomatous thyroiditis(1/29).1 The MVD count of benign thyroid nodules(24.00 ± 3.63)was significantly higher than that of malignant nodules MVD(17.27±3.11),and the MVD distribution of malignant nodules was not evenly distributed.In thyroid malignant nodules,the MVD count of the metastatic group(19.08±4.09)was higher than that in the non-metastaticgroup(16.57±2.24).2 In the thyroid malignant nodules,32 cases of VEGF staining positive expression(32/41),the positive expression of VEGF staining in the lymph node metastasis group(12/13)was higher than that in the non-lymph node metastasis group(20/28).3 There was a significant correlation between MVD count and VEGF expression in thyroid nodules,and the correlation coefficient r was 0.680.4 PI and TTP are positively correlated with MVD,where PI was significantly related to MVD,and the correlation coefficient r was 0.522,and TTP correlation coefficient r was 0.282,and there was no significant correlation between RT and MVD.5 There was a significant correlation between PI and VEGF expression in thyroid malignant nodule,and the correlation coefficient r was 0.407.There was no significant correlation between RT,TTP and VEGF.Conclusion:(1)routine thyroid nodule ultrasound can use boundary,morphology,presence of microcalcification,preliminary diagnosis of a CDFI,difficult for conventional ultrasound diagnosis of the nodules,tuberous nature combined with the characteristics of ultrasonic imaging for judgment.(2)quantitative analysis of benign and malignant thyroid ultrasonography has statistical significance in the difference of PI and lama PI,which may be helpful for the diagnosis of nodule properties.(3)the ultrasonographic parameters PI and TTP can reflect the MVD of thyroid nodules to a certain extent,providing pathological basis for the clinical diagnosis of thyroid nodules.(4)thyroid malignant nodule PI was significantly correlated with MVD and VEGF expression,which can provide information for thyroid malignant nodule prognosis evaluation.
