Objective:Explore tranexamic acid blood conservation function in patients with heart valve surgery and heart valve surgery in patients with preoperative,intraoperative and postoperative time gives dose tranexamic acid blood conservation effect in patients with cardiac valvular surgery.Method:120 patients undergoing cardiac valve surgery with Intravenous Inhalation Combined Anesthesia and extracorporeal circulation were randomly divided into preoperative medication group(group A),intraoperative drug delivery group(group B),postoperative medication group(group C)and preoperative medication control group(group D).A skin incision in group given tranexamic acid 2g Sodium Chloride Injection,group B in cardiac arrest,extracorporeal circulation when given tranexamic acid Sodium Chloride Injection 2g.In the group C,2g of sodium chloride injection was given when the thoracic suture was closed,saline in group D treated with skin incision.30 cases in each group.The amount of hemoglobin before and after operation,postoperative hemoglobin reduction,24 hours drainage volume,total drainage volume,and the use of red blood cells and fresh frozen plasma were recorded.Result:Compared with group D,group A after the whole hemoglobin concentration higher than D group,after 24 hours,the total drainage volume was lower than that of group D(p<0.05);Compared with group B,C,the hemoglobin content in group A was higher than that in group B and C(p<0.05).24 hours drainage volume,total drainage volume are low in the B and C group(p < 0.05),in the aspects of allogeneic blood use: compared with group D,amount of blood transfusion and transfusion rate lower than group A with statistical significance(p < 0.05),compared with group B and C,respectively,A set of amount of blood transfusion and transfusion rate is lower than that of group B,C,and statistically significant(p < 0.05).Conclusion:The results of this study suggest that:1.Tranexamic acid for blood protection effect of extracorporeal circulation in heart valve replacement is effective,can effectively reduce the perioperative blood loss,reduce the loss of hemoglobin,reduce the use of perioperative allogeneic blood,but can not reduce the operation time and cycle time in vitro.2.Under the safe dosage of preoperative administration of tranexamic acid on extracorporeal blood protection effect is better than that of postoperative medication,and no tranexamic acid related complications.
