Objective Through the survey of health status and health influence factors in Jinchang occupation cohort,it is to know the general life behavior、mental and psychological aspects and Helicobacter pylori infection condition;to observe the prevalence characteristic of Digestive inflammation and ulcers and crowd characteristics;to analyze the relationship among disease and factors and explore the influence factors.which is useful for the company to take a effective health promotion and disease prevention strategy.Methods 44944 workers in Jinchang Nickel exposure occupational cohort were chosen as the subject population.Through the survey of crowd health condition and health influence factor,we made the description of demographic information,Helicobacter pylori infection status and the prevalence rate of digestive inflammation and ulcers.A sample of 3283 workers were gotten through stratified random sampling method for the further research.Then the statistical approach of Chi-square test,binary logistic regression and log-linear regression were used to explore the interaction effect and influence factors.Results 1.The prevalence rate of chronic digestive inflammation and ulcers is 9.24%,the rate of male is 9.83%,and the rate of female is 8.33%.The digestive inflammation including chronic gastritis,chronic rectitis and reflux esophagitis accounts for 81.3%.The disease orders are chronic gastritis,chronic rectitis,gastric ulcer,duodenal ulcer and reflux esophagitis and the prevalence rates is 6.46%,1.84%,1.24%,0.63%and 0.17%respectively.2.The results shows that preferring smoked foods(OR=11.006,95%CI=(1.998~20.627),dust exposure(OR=1.446,95%CI=1.047~1.998)and LDL-C(OR=1.298,95%CI=1.064~1.582)are independent risk factors of digestive inflammation and ulcers.Besides,the interaction between age and repression,age and preferring thin-bodied foods,adds to the prevalence.The specific effects are:over the age of 60 and with repression(OR=2.760,95%CI=1.614-4.720),50~59 years old and with repression(OR=2.585,95%CI= 1.630-4.099),40-49 years old and with repression(OR=2.505,95%CI=1.724~3.640),30~39 years old and with repression(OR=1.925,95%CI=1.262~2.935);50~59 years old and prefer thin-bodied foods(OR=2.875,95%CI=1.286~6.428),40~49 years old and prefer thin-bodied foods(OR=1.962,95%CI= 1.045~3.687).3.The results shows that preferring smoked foods(OR=2.682,95%CI=1.977~7.363),age(OR=2.582,95%CI=1.347-4.950)and repression(OR=1.871,95%CI=1.160-3.016)are inde-pendent risk factors of chronic digestive inflammation.Besides,the interaction in lower cultural degree,dust exposure and serum levels of Low density Lipoprotein Cholesterol adds to the prevalence.the specific two kinds of people are easier to sick:one kind is the man who with lower cultural degree and espousing to the dust,at the same time,the serum levels of LDL-C are higher than 95%normal crowd(OR=1.818,95%CI=1.100~3.005).the other kind is the man who with lower cultural degree and espousing to the dust,at the same time,the serum levels of LDL-C are the normal value of 95%crowd(OR=1.516,95%CI= 1.034~2.224).4.The results shows that preferring smoked foods(OR=10.568,95%CI=1.655~20.133)is the independent risk factor of Peptic ulcer.the interaction between age and diet temperature adds to the prevalence.the specific effects are:50~59 years old and preferring the hot foods(OR=6.897,95%CI=4.970~9.969),over the age of 60 and preferring the hot foods(OR=5.984,95%CI=3.068~7.526).Conclusions 1.The intake rate of meat,processed and smoked food are high in this occupational population;In male population,especially age between 30 and 39,the rates of smoking and drinking are higher than other age group;The crowd spirit is depressed clearly.2.The detection rate of Helicobacter pylori in this occupational crowd is different with the national average level,and is low from a general view.Among various kinds of diseases,the peptic ulcers have a higher detection rate.3.The crude prevalence rate of chronic digestive inflammation and ulcers is high,however,after standardization,the rate of all kinds of disease is lower than average level.In addition,the prevalence rate of digestive inflammation is ranked first.The female seem to be the high-risk group of chronic gastritis,and chronic rectitis and peptic ulcer are more likely to develop in male population.4.The results indicate that preferring thin-bodied foods and repression are closely related to digestive inflammation,smoked food is the important risk factor for digestive ulcer,and the prevalence rate of this disease is closely related with dust exposure.In addition,preferring thin-bodied,hot foods and LDL-C content also can act on the disease thorough interaction.The company should propose healthy dietary habit pay attention to worker mental health,and on the basis of good enterprise dust prevention measures,improve the use of individual dust proof measures.
