| Objective:To investigate the correlation between TCM Constitution of patients with gastric cancer and serum inflammatory cytokines such as GM-CSF,IL-5,IL-7,IL-1α,IL-12/IL-23p40,IL-15,IL-16,IL-17α,TNF-β,VEGF,IFN-γ,IL-1β,IL-2,IL-4,IL-6,IL-8,IL-10,IL-12p70,IL-13,TNF-α.Methods:135 patients with gastric cancer who were visiting the Department of oncology,Jiangsu Province Hospital of Traditional Chinese Medicine,from March 2014 to December 2015,were enrolled in this study.92 physical examinees who were visiting the physical examination center,Jiangsu Province Hospital of Traditional Chinese Medicine,from April 2015 to December,were also enrolled in this study.Healthy control group and gastric cancer patients are matched in gender and age.According to "Classification and Determination of Constitution in TCM Association of traditional Chinese Medicine,the subjects were divided into nine types,respectively is Balanced constitution,Qi-deficient constitution,Yang-deficient constitution,Yin-deficient constitution,Phlegm-dampness constitution,Dampness-heat constitution,Blood-stasis constitution,Qi-stagnation constitution and Special constitution,and physical fitness score for all subjects,the relationship between TCM Constitution of patients with gastric cancer and serum inflammatory cytokines were analyzed by SPSS 17 statistical software.Result:1.A total of 135 cases of patients with gastric cancer,aged from 33 to 84,with an average of(63.89±9.37)years old,of which there were 108 cases of male patients and 27 cases of female patients.A total of 92 cases of physical examinees,aged from 31 to 84,with an average of(63.28±11.0)years old,of which there were 74 cases of male physical examinees and 18 cases of female physical examinees.Because a patient probably has a variety of constitutions,we need to carry on the frequency statistics of them.The total frequency in the case group was 210,balanced constitution accounted for 38.1%(80/210),eight kinds of biased constitution accounted for 61.9%(130/210).In the biased constitution,from high to low:Yang-deficient constitution accounted for 19%(40/210),Qi-deficient constitution accounted for 17.1%(36/210),Qi-stagnation constitution accounted for 7.1%(15/210),Yin-deficient constitution accounted for 5.7%(12/210),Blood-stasis constitution accounted for 3.8%(8/210),Special constitution accounted for 3.3%(7/210),Phlegm-dampness constitution accounted for 2.9%(6/210),Dampness-heat constitution accounted for 2.9%(6/210).The total frequency in the control group was 207,eight kinds of biased constitution accounted for 71.5%(148/207).In the biased constitution,from high to low:Yang-deficient constitution accounted for 12.5%(26/207),Blood-stasis constitution accounted for 12.5%(26/207),Yin-deficient constitution accounted for 10.6%(22/207),Qi-deficient constitution accounted for 9.18%(19/207),Phlegm-dampness constitution accounted for 9.18%(19/207),Dampness-heat constitution accounted for 9.18%(19/207),Special constitution accounted for 5.2%(11/207),Qi-stagnation constitution accounted for 4.3%(9/207).There were differences in the constitution structure between the control group and case group.Case group showed that its rate of Blood-stasis constitution and Yin-deficient constitution were decreased,but rate of Qi-deficient constitution and Yang-deficient constitution were increased,and showed that constitution types of case group were mainly Qi-deficient constitution and Yang-deficient constitution.2.The results of the correlation between gender of patients with gastric cancer and serum inflammatory cytokines showed that,compared with male examinees of the control group,the expression level of IFN-γ and IL-6 in serum were decreased in male patient of the case groups,compared with female examinees of the control group,the expression level of IFN-γ and IL-6 in serum were increased in female patient of the case groups,indicating that IFN-γ,IL-6 in serum of patients with gastric cancer of different genders were different in the state of inflammation.The results of the correlation between age of patients with gastric cancer and serum inflammatory cytokines showed that,in the case group,compared middle-aged patients with young patients,the expression level of IFN-γ in serum was decreased,in the control group,compared middle-aged persons with young persons,the expression level of IFN-γ in serum was increased.In the case group,compared elderly patients with middle-aged patients,the expression level of IL-2,IL-6 in serum were decreased,in the control group,compared elderly persons with middle-aged persons,the expression level of IL-2,IL-6 in serum were increased,indicating that IFN-γ,IL-2,IL-6 in serum of patients with gastric cancer of different age were different in the state of inflammation.Compared youngsters of the case group with the control group,the expression level of GM-CSF in serum was decreased,compared middle-aged people of the case group with the control group,the expression level of GM-CSF in serum was increased,compared the elderly of the case group with the control group,the expression level of GM-CSF in serum was decreased,indicating that GM-CSF in serum of patients with gastric cancer of the middle-aged state is different from the young and elderly patients in the state of inflammation.Compared youngsters of the case group with the control group,the expression level of IFN-γ in serum was increased,compared the elderly of the case group with the control group,the expression level of IFN-γ in serum was decreased,indicating that IFN-γ in serum of patients with gastric cancer in the youth and the elderly were different in the state of inflammation.Compared middle-aged people of the case group with the control group,the expression level of TNF-β in serum was decreased,compared the elderly of the case group with the control group,the expression level of TNF-β in serum was increased,indicating that TNF-β in serum of patients with gastric cancer in the wrinkly and the elderly were different in the state of inflammation.3.The effect of surgery and chemotherapy on the level of serum inflammatory cytokines in patients with gastric cancer showed that most patients with gastric cancer had undergone surgery and chemotherapy.Compared with nonoperative patients,the expression of IL-16 and VEGF in patients with surgery were decreased,indicating that surgery affected the expression levels of IL-16 and VEGF in patients with gastric cancer.Compared with non chemotherapy patients,the levels of GM-CSF,IL-1α,TNF-β,IFN-γ,IL-2,IL-6,IL-10 and IL-12p70 in patients with chemotherapy were decreased,indicating that chemotherapy had an influence on the expression of various inflammatory cytokines in patients with gastric cancer.4.The results of the correlation between nine types TCM constitution of patients with gastric cancer and serum inflammatory cytokines showed that in the case group,compared Qi-deficient constitution with Dampness-heat constitution,the expression level of IL-6 in serum was increased;in the control group,compared Qi-deficient constitution with Dampness-heat constitution,the expression level of IL-6 in serum was decreased,indicating that IL-6 was closely related to the formation of Qi-stagnation constitution of patients with gastric cancer.In the control group,compared Blood-stasis constitution with Balanced constitution,the expression level of TNF-β in serum was increased,and in the case group,compared Blood-stasis constitution with Yin-deficient constitution,the expression level of TNF-β in serum was decreased.Compared Blood-stasis constitution of the case group with the control group,the expression level of TNF-β in serum was decreased,indicating that TNF-β was closely related to the formation of Blood-stasis constitution of patients with gastric cancer.Conclusion:1.There were differences in the TCM constitution structure between the control group and case group.Healthy control group showed that its biased constitutions were Yang-deficient constitution,Blood-stasis constitution and Yin-deficient constitution.Case group showed that its rate of Blood-stasis constitution and Yin-deficient constitution were decreased,but rate of Qi-deficient constitution and Yang-deficient constitution were increased,and showed that constitution types of case group were mainly Qi-deficient constitution and Yang-deficient constitution.2.There were differences in regulating network of serum inflammatory cytokines between different TCM constitution of patients with gastric cancer.There was high complexity of the mutual regulation of serum inflammatory cytokines of Balanced constitution,and low complexity of the mutual regulation of serum inflammatory cytokines of unbalanced constitution,which indicated that either type of constitutions showed their own important correlation rule.3.Cytokines such as IL-13 of Yang-deficient constitution,TNF-β of Blood-stasis constitution,IL-8 and TNF-α of Qi-stagnation constitution probably had correlation with specificity cytokines in patient with gastric cancer such as IL-13,TNF-β,IL-8 and TNF-α.4.Chemotherapy really affect the expression of serum inflammatory cytokines such as GM-CSF,IL-1,TNF-β and IFN-γ,IL-2,IL-6,IL-10 and IL-12p70 in patients with gastric cancer.5.Serum inflammatory cytokines may be the key factor in the formation of biased constitution of patients with gastric cancer. |
