Objective:To assess the value of quantitative dynamic contrast enhanced MRI in diagnosing the benign and malignant lesions of breast disease from the following parts:lesion morphology,time-signal intensity curve(TIC)typing and quantitative parameters.Materials and Methods:Twenty-one patients with breast disease admitted in our hospital from December 2014 to January 2017 were retrospectively selected.Among them,13 patients were breast cancer patients as malignant group and 8 patients with benign breast disease as benign group.All patients were received preoperative 3.0T DCE-MRI exanination and postoperative pathological examination.First,analyze the lesion morphology;second,draw time-signal intensity curve(TIC)and estimate the typing;Last,calculate the quantitative parameters including:volume transfer constant(Ktrans),reverse volume transfer constant(Kep),blood plasma volume per unit volume of tissue(Vp),etc,and analyze the benign and malignant lesions of the breast statistically.Results:After dynamic contrast-enhanced MRI,in lesion morphology,the sensitivity of diagnosing breast cancer is 84.62%(11/13),the specificity is 75.00%(6/8),the accuracy is 80.95%(17/21);in time-signal intensity curves,the sensitivity of diagnosing breast cancer is 92.31%(12/13),the specificity is 62.50%(5/8),the accuracy is 80·95%(17/21);quantitative DCE-MRI proves that the mean Ktrans,Kep,and Vp of malignant lesions were(10.4571±7.4720)min-1,(1.2147±1.5284)min-1 and(0.15921±0.1672)ml;the mean Ktrans,Kep,and Vp of benign lesions were(4.2206±4.0238)min-1,(2.3387±1.6586)min-1 and(0.2628±0.1969)ml.The Ktrans were statistically significant(P<0.05)between malignant and benign group,while Kep and Vp were not statistically significant(P>0.05).Ktrans diagnoses breast cancer with a sensitivity of 76.9%,and a specificity of 87.5%.Conclusions:DCE-MRI images showed that irregular shape,spicules of margin and Ⅲ-type curve are the typical imaging findings for breast cancer;morphological rounded or rounded,smooth margins are the imaging features of benign breast disease,Ⅰ-type curve for breast benign disease has a certain significance,and Ⅱ-type curve can be seen in both benign and malignant group which cannot be differential diagnosis of benign and malignant.Quantitative DCE-MRI showed that Ktrans parameters were statistically significant for the identification of benign and malignant breast diseases,while the Kep and Vp parameters were not statistically significant for the differential diagnosis of benign and malignant breast diseases.
