Analysis And Comprehensive Evaluation Of Clinical Pathway Management In A Tertiary Hospital Based On Comprehensive Index Method

ObjectiveIn order to analyze the status quo of clinical pathway management in a tertiary hospital objectively and comprehensively,this paper depicted the development and changes of clinical path implementation in the past three years.According to problems inside,analyzed the managerial experience in clinical pathway which may give the reference for the health administration and hospital managers to promote more comprehensive and delicacy management of clinical pathway.MethodsAccording to literature review,we understood the current research status of domestic and international clinical pathway management and did the research on the hot spots of new medical reform.Through the Clinical Pathway Newsletter issued by Shanghai Municipal Health and Family Planning Commission in 2015 and 2016,and with the data from the medical association information platform in 2014 to 2016,we used weighted comprehensive index method to evaluate the clinical pathway of the hospital.ResultsBy the status quo of clinical pathway management,the hospital has implemented the clinical pathway since 2011,the number of disease types and pathways has increased by years,and the full coverage of disciplines is realized.The number of completed clinical pathways has also increased year by year,and the growth rate has reached 352.22% in2015.The percentage of completed clinical pathways cases in hospital discharges showed a year-on-year growth,that maintained a high growth rate during 2014-2016;the number of completed clinical pathways ranked first in 2015 among Shanghai-level general hospitals and ranked second in 2016.The percentage of completed clinical pathways cases in hospital discharges was ranked first in 2015 and ranked second in 2016,which were significantly higher than the average of Shanghai-level general hospitals;the average hospitalization days was the third lowest in 2015,significantly lower than the average of municipal-level general hospitals.In 2016,the average length of stay dropped to the sixth place,which is slightly lower than the average level of municipal-level general hospitals in2015 and 2016.The clinical pathways' entry rate and completed rate in this hospital were higher than other municipal hospitals in Shanghai,and the average mutation rate was lower than those hospitals.According to the quality of the implementation of clinical pathway,it shows anincreasing trend which marked 8.6500,9.2029,and 9.9380 during 2014-2016.ConclusionAs a large-scale tertiary hospital,it occupied a dominant position in the Shanghai-level general hospitals after years of practice and exploration.The implementation quality of the clinical pathway has been increasing year by year.However,there still leave many rooms to improve,so the steps should be taken for the further quality improvement in the area of clinical pathway.
