Objective:To explore the correlation between TCM syndrome elements and menstrual abnormalities and sex hormone levels in PCOS(polycystic ovary syndrome),so as to provide references for diagnosis and treatment of TCM in this disease.Methods:Set PCOS patients as observing object.According to their menstrual conditions,the patients are divided into three groups as oligomenorrhea,irregular menstrual and secondary amenorrhea.Standardized four diagnostic information of each patient in all groups are collected by using "four diagnostic information table",and their height,weight,waist circumference,hip circumference,serum hormone levels are recorded at the same time.The distribution characteristics of TCM syndromes in each group and differences of syndrome hormone and serum hormone between the groups are analyzed after the four diagnostic information calculated into syndrome elements in identification system.Results:1.Characteristics of TCM syndrome elements of disease location in PCOS:The top seven diseases location are liver,uterus,spleen,kidney,stomach,skin and mind,liver and uterus are the most(P<0.01),followed by spleen and kidney,which are significantly higher than that of skin and heart(P<0.01 or P<0.05).2.Characteristics of TCM disease syndrome elements in PCOS:The top five diseases are heat,Qi stagnation,blood stasis,Yin deficiency and blood deficiency.The frequency of heat and Qi stagnation are significantly higher than those of other syndromes(P<0.01).No difference in frequency of blood stasis,Yin deficiency and blood deficiency.3.Characteristics of syndrome elements of disease location in three groups:In oligomenorrhea and irregular menstrual,liver and uterus are the most,both are significantly higher than other syndromes(P<0.01);In secondary amenorrhea,liver is significantly higher than that of other syndrome(P<0.01),no difference in frequency of uterus,spleen and kidney.4.Characteristics of disease syndromes in three groups:Excess syndrome:In oligomenorrhea and irregular menstrual,heat,Qi stagnation and blood stasis are significantly higher than those of other syndromes(P<0.01);In secondary amenorrhea,heat,Qi stagnation and phlegm are most(P<0.05).Deficiency syndrome:No difference between Yin deficiency,blood deficiency and Qi deficiency in oligomenorrhea and irregular menstrual,and the difference between Yin deficiency and Yang deficiency is significant(P<0.01).No difference between the deficiency syndrome in secondary amenorrhea;The constituent ratio of blood deficiency in secondary amenorrhea is significantly higher than that of the other two groups.5.Characteristics of mixed syndrome in three groups:Mixed disease locations:uterus with liver is the most in each group(P<0.01).Mixed excess syndrome:In oligomenorrhea and irregular menstrual,Qi stagnation,heat and blood stasis are most with each other(P<0.01 or P<0.05);In secondary amenorrhea,Qi stagnation,heat and phlegm are most with each other(P<0.01 or P<0.05).Mixed deficiency syndrome:In oligomenorrhea,deficiency of Qi and Yin is most,significant difference compared with deficiency of Yin and Yang(P<0.05);No difference in irregular menstrual and secondary amenorrhea.Mixed deficiency and excess:The mixed deficiency and excess is the most in each group,and significant higher than deficiency syndrome and excess syndrome(P<0.01).6.Correlation between syndrome elements and BMI,WHR in three groups:The phlegm is significant positively correlated with BMI in each group(P<0.01),and which is the highest in secondary amenorrhea(r=0.661).In oligomenorrhea and irregular menstrual,dampness and BMI is positively correlated(P<0.05);In secondary amenorrhea,spleen is positively correlated with BMI(P<0.05).7.Correlation between syndrome elements and serum sex homone in three groups:No statistical difference between serum hormone levels in each group.In oligomenorrhea,no significant correlation between syndrome elements and sex hormone.In irregular menstrual,liver,uterus,Qi stagnation and blood stasis are positively correlated with T(P<0.05);The kidney is positively correlated with E2(P<0.05).In secondary amenorrhea,kidney is positively correlated with LH(P<0.05).Heat is positively correlated with T(P<0.05);There is a positive correlation between uterus,Yang deficiency and PRL(P<0.05);The spleen,phlegm and dampness are negatively correlated with E2(P<0.05).Conclusions:1.The main disease location of PCOS are liver and uterus,followed by spleen and kidney.2.The syndrome of PCOS belongs to mixed deficiency and excess,the main disease syndrome are Qi stagnation and heat;blood stasis,phlegm,Yin deficiency and blood deficiency have important effect on the occurrence and development of PCOS.3.Blood stasis is an important pathological factor in oligomenorrhea and irregular menstrual of PCOS.4.Spleen and kidney are closely related to secondary amenorrhea in PCOS,and blood deficiency and phlegm are the important cause of secondary amenorrhea.5.The level of sex hormone can reflect the degree of abnormal menstruation and the characteristics of TCM syndrome to some extent.
