Objective:Comparative study of postoperative visual quality of aspheric intraocular lens and spherical intraocular lens implanted in patients with high myopia and cataract.Methods:A Prospective non-randomized controlled study collected high myopia with cataract patients who were admitted to the First Affiliated Hospital of Hunan Normal University from January 2017 to January 2018.A total of 51 patients(63 eyes)were divided into two groups.Group A had 32 eyes.Aspherical intraocular lens was implanted intraoperatively.Group B had 31 eyes.Spherical intraocular lens was implanted during operation.cataract phacoemulsification and IOL implantation were performed by the same experienced eye ophthalmic cataract surgery group physician.All selected cases were preoperatively One day after operation,one week after operation,one month after operation,and three months after operation were measured by the international standard visual acuity meter.、optical quality analysis system(OQAS)。compare the uncorrected visual acuity(UCVA),best corrected visual acuity(BCVA)and objective scattering index(OSI),modulation transfer function(MTF)cut off frequency(MTF cut-off value),Strehl ratio(SR),OQAS when the contrast ratio is 100%,20%,and 9%,respectively Values(OQAS value,OV100%,OV20%,OV9%)were assessed by subjective visual function and quality of life(VF and QOL)questionnaires in both groups at 3 months postoperatively.Results:(1)In group A,32 eyes were included for eye surgery,and 31 eyes were included in group B.The uncorrected visual acuity and best corrected visual acuity of the two groups were improved compared with those before operation.The visual acuity of the eyes was 1 month after surgery.The best-corrected visual acuity stabilized,and there was no significant difference between the uncorrected visual acuity and best corrected visual acuity in group A at 1 day,1 week after surgery,and 3 months after surgery compared with those in group B(P>0.05).(2)The values of MTF cut-off,OSI,SR,OV100%,OV20%,and OV9%were not significantly different between the two groups before operation(P>0.05).The MTF cut-off value was increased gradually at 1 week after surgery.The OSI values at each time point in both groups were gradually decreased compared with before surgery.The SR values at 1 day and 1 week after surgery were both decreased in both groups.Compared with the preoperative increase,all indexes were stable after 3 months;A group was postoperative 1 day(0.13±0.01),postoperative 1 week(0.15±0.02)and group B postoperative 1 day(0.12±0.01)The SR values were statistically significant(P>0.05)in patients(0.14±0.02)at 1 week after surgery(P<0.05).The A group was postoperative 1 day(18.82±3.50),postoperative 1 week(22.35±1.92)and group B.There was a significant difference in MTF cut-off at 1 day(16.53±4.32)and 1 week(21.49±1.37)postoperatively(P>0.05).At each time point and at the same time point in group A,the difference was statistically significant(P>0.05).OSI values in group B were statistically significant(P>0.05).All patients were statistically insignificant in postoperative 1 day,postoperative 1 week,postoperative 3 month,and preoperative OV100%,OV20%,and OV9%values.Significance(P>0.05),Group A patients There was no statistically significant difference in OV100%,OV20%,and OV9%between the first day after surgery,one week after operation,one month after operation,three months after operation,and at the same time in group B(P>0.05).(3)3 months after operation,VF/QOL questionnaire showed that there was a significant difference in visual acuity(P<0.05)and stereo perception(P<0.05)between group A and B(P<0.05).There was no significant difference in subjective vision,peripheral vision,self-care,activities,social and psychological aspects(P>0.05).Conclusions:1.After phacoemulsification and intraocular lens implantation in cataract patients with high myopia,both aspherical lenses and spherical intraocular lenses can improve the visual acuity of patients.2.Optical Quality Analysis System and visual function quality of life indicated that the visual quality of aspherical intraocular lenses is better than that of spherical intraocular lenses.3.Optical Quality Analysis System can objectively evaluate the visual quality,and can perform quantitative analysis,the combined subjective visual function quality of life questionnaire has a positive guiding significance in assessing the visual quality of postoperative intraocular lens implantation.
