Objective:Objective to compare the clinical effects of paraspinal approach and posterior median approach in the treatment of lumbar fracture without nerve injury.Methods:A retrospective analysis was carried out from 01 months to December 2016 from 2014 to December 2016 in our hospital.One hundred and seventeen cases of the posterior approach and paravertebral muscle approach were performed without nerve injury.According to the operative approach,they were divided into paraspinal approach group(group A,57 cases)and posterior median approach group(group B,60 cases).Two groups of operation time,intraoperative bleeding amount and infection rate were collected before and after operation,and the VAS score,JOA score,vertebral height loss rate,wedge angle,and CT paravertebral muscle area were collected through case records and follow-up,and SPSS 18.0 software analysis(P>0.05 was not statistically significant,P<0.05 was statistically poor.The chi square test compared two groups of sex,fracture segment,cause of injury and infection.Two groups of independent samples t test compared two groups of age,operation time,bleeding volume,VAS score,JOA score,vertebral height loss rate,vertebral wedge angle,CT paravertebral muscle area in different intervertebral segments.Results:All cases were followed up for more than 1 years.There was no significant difference between the two groups in general condition(sex and age)and preoperative clinical data.Two groups of independent samples t test showed that two groups of operation time and intraoperative bleeding were statistically significant(P<0.05);chi square test showed no statistical significance(P>0.05);two groups of independent samples t test two group preoperative vertebral height loss rate,vertebral wedge angle,preoperative VAS,preoperative JOA score,preoperative CT vertebra There was no significant difference in the area of paravertebral muscles(P>0.05),and there were statistically significant differences between the two groups at different periods of time,JOA scores in different periods,and the area of CT paravertebral muscles at different periods after the operation(P<0.05).Conclusion:Paravertebral approach has the advantages of short operation time,less intraoperative bleeding,less postoperative pain,good recovery of lumbar function,and paravertebral muscle protection in the treatment of single segment lower lumbar fracture without nerve injury.
