| Purpose Discuss the clinical application value of Fibrotouch in patients with fatty liver: assess the consistency of Fibrotouch and B-ultrasound in the diagnosis of fatty liver disease;study the related factors affecting fat attenuation parameters of fatty liver;analyze liver fibrosis and risk factors in patients with fatty liver.Methods A retrospective collection of 181 patients who were admitted to the First Affiliated Hospital of Nanjing Medical University from January 2017 to January 2018 for Fibrotouch examination and B-ultrasound examination.Kappa test was used for consistency analysis and Fibrotouch was used to detect FAP and LSM of patients` livers.Multivariate linear regression analysis was used to establish a regression equation.Biochemical indicators,as well as age,sex,and BMI were obtained and multiple linear regression analysis was performed to establish regression equations.To evaluate the detection rate of hepatic fibrosis in fatty liver patients with liver stiffness(LSM)and liver fibrosis related indicators,including NAFLDFS APRI and FIB-4.The difference between F0-2 and F3-4 was compared.Results 1.In this study,a total of 181 patients were included,122 males and 59 females.The male to female ratio was 2.07:1 and the average age was 45.81±14.57 years.BMI was 24.68±3.60 kg/m2 and fat loss was 257.25±38.10 db/m and liver hardness was 17.785 ± 14.31 k Pa.Kappa of this two detection methods was 0.711 and the total compliance rate was 85.64%.2.According to the univariate analysis of 60 patients with NAFLD diagnosis,there was a positive correlation between fat attenuation and BMI,TG,and GGT.The P value was less than 0.001,which was statistically significant.Using multiple stepwise regression method for regression analysis,we can see that GGT,TG,BMI three indicators entered the regression model and P values were less than 0.05;The numerical model of the regression equation obtained from the model results is: Y=-41.470+2.209*GGT+8.947*TG+0.171*BMI.Fat attenuation was linearly correlated with BMI,GGT,and triglycerides.3.Among the 60 patients with NAFLD,there was no statistically significant difference in liver fibrosis rates according to the indicators of LSM,NAFLDFS,APRI,and FIB-4.The age,BMI,ALT and AST levels in F3-4 were higher than those in F0-2,while albumin in F3-4 was lower.Sex ratio composition,triglycerides,total cholesterol,GGT,ALP were not statistically different between the two groups.Logistic regression analysis showed that BMI(OR:1.536,95%CI:1.126-2.096)ALT(OR:1.088,95%CI:1.030-1.149)and AST(OR:1.065,95%CI:0.997-1.137)were positively correlated with liver fibrosis,indicating that obesity,high ALT levels,and high AST levels were risk factors for liver fibrosis in patients with fatty liver.Conclusions Fibrotouch and B-ultrasound had higher conformance and good consistency in the diagnosis of fatty liver;Fat attenuation parameters in NAFLD patients were affected by GGT,triglyceride,and BMI;BMI attenuation parameters in NAFLD patients were linearly related to BMI and liver stiffness. |
