| Background:The gallbladder,as an organ that enrichment、stored and discharge of bile,is an important component of the biliary system.Chronic Calculous Cholecystitis is a biliary dyskinetic disease.Gallbladder power decline will make the gallbladder bile discharge function is impaired,leading to bile siltation,increase degree of gallbladder inflammation.In cholelithiasis,gallstone long-term stimulation gallbladder mucous membrane has the inflammation,and further led to a decline in the gallbladder power[1].Studies have shown that enteric nerve-ICC-smooth muscle network is the basic unit of gastrointestinal motility function[2].In 2007,Hinescu ME et al[3]finally confirmed the existence of interstitial cells of cajal(ICC)in Disease-free gallbladder of patients.In2007,The ICC was also be founded in the gallbladder and extrahepatic bile duct of guinea pig by Huang et al[4].All of these confirmed that in the extrahepatic bile duct system and gallbladder was existing neural-Cajal interstitial cells-smooth network.The discovery of neural-icc-smooth muscle networks has pointed out a new direction for the study of biliary dyskinetic disorders,and provides a new theoretical basis for the treatment of Chronic Calculous Cholecystitis.The common treatment models of Chronic Calculous cholecystitis include surgical treatment,and medical therapies.Generally asymptomatic chronic calculous cholecystitis patients with drug therapy,these patients often in physical examination was found to suffer from this disease.The recurrent Chronic Calculous cholecystitis should be use cholecystectomy to treatment.In recent years,with the strong support of the state for the development of traditional Chinese medicine,some diseases have started to be treated with traditional Chinese medicine.Among them,Yinchenhao Decoction has achieved good results in the treatment of chronic stone cholecystitis.Yinchenhao Decoctions is from the chapter of Yangming disease in Zhang Zhongjing’s"Treatise on Febrile Diseases".This medication is composed of artemisia capillaris,fructus gardeniae,and rhubarb;which have functions of clearing away heat,removing dampness by dieresis.Clinical application confirmed that Yinchenhao Decoction has significant effect on the treatment of chronic calculous cholecystitis.Our early study found[8]the ICC increased significantly when the Yinchenhao Decoctions was used in the treatment of chronic calculous cholecystitis,and achieve the goal of ease the gallbladder dynamic obstacles.In order to further explore the mechanism of the drug treatment of chronic calculous cholecystitis,we need through further research on a cellular level.Objective:By observing the guinea pig gallbladder inflammation levels,nerve-the ICC-smooth muscle morphological structure and the changes of the ultrastructure of network before and after the treatment,to discusses the relationship between the degree of gallbladder inflammation and the ICC.And the effect of Yinchenhao decoction on gallbladder neuro-ICC-smooth muscle network in guinea pig model with cholesterol gallstone.Methods:Select experiment male guinea pigs with regular grade,3 to 4 months of age,weight is about 200±50 grams,randomly divided into normal group,model group and treatment group.After the breeding and treatment,the gallbladder tissue of each group were prepared for HE staining,MPO kits,immunofluorescence test and transmission electron microscope to observe the ultrastructure.Results:1.HE staining showed that the model group’s gallbladder mucosal epithelial cells arranged in disorder,there are some epithelial shedding,muscularis edema compensatory hypertrophy concurrent existing a large number of lymphocyte invasion.The control group is normal.The treatment group compared with model group significantly improved.2.MPO kits,the light absorption luminosity values and MPO activity of treatment group is lower than the model group and higher than the control group.3.Immunofluorescence results showed that compared with the normal group,the fluorescence intensity of the model group decreased significantly.The fluorescence intensity in treatment group is increased significantly than model group.4.Tem observation results show that in the model groups,the protuberances of ICC cells reduced significantly,the gap between cells were increased.In the normal group,ICC contains abundant mitochondria,endoplasmic reticulum,and well developed golgi apparatus,ICC nerve cells and smooth muscle cells were connected with gap-like junction,which was the main connection mode.Treatment group the ICC cells swelled damage is not apparent,the ICC to each other and there is a gap junction between the ICC and smooth muscle cells.Conclusion:1.Treatment with Yinchenhao decoction can reduce the cholesterol calculus model of guinea pig gallbladder inflammation,Increase the expression of the number of ICC cells.2.The number of ICC cells in the gallbladder of the model group was reduced and the cell structure was damaged.The protuberances of ICC cells reduced significantly,the gaps between cells were increased and the number of gap junction was reduced.The nerve-ICC-smooth muscle network of the gallbladder was damaged,the treatment of Yinchenhao Decoction can improve this situation.3.Through the analysis of experimental results shows that,Yinchenhao Decoction can get obvious effect in the treatment of chronic calculous cholecystitis. |
