Background:With the relationship between tumor and immunity has been found,and since the role of tumor microenvironment in tumor invasion and metastasis in the deepening of the research,a variety of mechanisms leading to immune cell dysfunction in the tumor microenvironment have been found that immune cells(such as lymphocytes and monocytes)can affect the biological characteristics the tumor cells,thereby affecting the occurrence and development of tumor.So many immune cells can be used to judge the prognosis of malignant tumor,especially the lymphocyte.However,lymphocyte / monocyte ratio(LMR)is the most important factor in the clinical study of immune cells and the prognosis of malignant tumors.Lymphocytes and monocytes play an important role in the process of tumor initiation and development as an important component of immune system and tumor microenvironment.At present,a large number of literatures have confirmed that LMR is an independent risk factor for the prognosis of multiple malignancies..Pancreatic cancer is a common malignant tumor in the digestive system with high malignancy,and the postoperative median survival time was 13.3 months.The low rate of early detection,and high rate of metastasis and recurrence after operation are the main clinical characteristics of Pancreatic cancer,even though there is no abnormalities found in imaging examination,most cases of pancreatic cancer patients diagnosed with subclinical liver metastases.The 5 year survival rate of pancreatic cancer is about 5%.The prognosis of pancreatic cancer is poor.It is a worldwide refractory malignant tumor.At present,the prognosis of pancreatic cancer is mainly judged by pathological type and clinical stage.In clinical work,there have been manystudies show that peripheral blood LMR can determine the prognosis of breast cancer,lung cancer,lymphoma and other malignancies,and the domestic research of LMR in the prognosis of patients with pancreatic cancer is relatively small,this study is to retrospectively analyze the relationship between LMR and the prognosis of patients with pancreatic cancer before surgery peripheral in the blood,hoping to find a simple and hematological index to judge the prognosis of patients with pancreatic cancer from the clinical point of view,so as to guide the clinical,as to prolong overall survival in patients with pancreatic cancer as possible,which can improve the prognosis of patients with pancreatic cancer.It is also hoped that this study can provide guiding indicators for preoperative staging of pancreatic cancer,provide a basis for the judgement of immune function of pancreatic cancer patients,and also provide some basic indicators for patients to regulate immune therapy.Aim:Preliminary study on the correlation between peripheral blood LMR and the prognosis of pancreatic cancer.Method:Collect 2006-01 to 2016-12 Qingdao municipal hospital confirmed by pathology in80 cases of LMR,and the age of patients,CEA,CA199,albumin,pathological type,histological pancreatic cancer patients with peripheral blood Ki-67 grade,and OS data;the receiver operating curve(ROC)analysis of the best diagnosis of LMR in patients with pancreatic cancer value(cut off);analysis of LMR and pancreatic cancer clinical pathological features(age,CA199,albumin,pathological type,histological grade,Ki-67,OS)of the relationship;using the Cox proportional hazards model to analyze the influencing factors on the prognosis of patients with pancreatic cancer.Result:According to ROC curve analysis,the best diagnostic value of LMR was2.82.Eighty patients were divided into low LMR group and high LMR group,the median total survival period of the high LMR group and the low LMR group was 24 months and 2 months,respectively.LMR had no correlation with age,CEA and albumin,and was correlated with pathological type,histological grade,Ki-67 valueand CA199 value.Conclusion:LMR is an independent risk factor affecting the prognosis of pancreatic cancer patients,it can be used as an maker for prognosis evaluation of pancreatic cancer patients.
