Clinical Study On The Correlation Between Visfatin,RBP4 And SFRP5 Levels In Patients With Type 2 Diabetes Mellitus Combined With Non-alcoholic Fatty Liver Disease

Objective:To observe the clinical effect of Compound Gegen Qinlian Decoction on type 2 diabetes mellitus(T2DM)combined with non-alcoholic fatty liver disease(NAFLD),to evaluate the clinical efficacy of Compound Gegen Qinlian Decoction and to explore its possible mechanism.Methods: The 60 patients who with T2 DM and NAFLD were divided into treatment group and control group randomly,each with a total of 30 cases.The control group was given conventional treatment of hypoglycemic and lipid regulating.On the basis of the control group,the treatment group was given Gegen Qinlian Decoction for 8weeks.Compare two groups before and after treatment in patients with BMI,WHR,FPG,Hb A1 C,2h PG and FINS,HOMA-IR,TC,TG,LDL-C,HDL-C,ALT,AST,GGT,Visfatin,RBP4,SFRP5 index and the change of TCM syndrome integral,parallel security inde-x detection.Results:(1)Clinical efficacy comparison: the total effective rate of the treatment group was 90%,and the control group was only 66.7%,and the difference was statistically significant(P<0.05).(2)Comparison of TCM syndromes: after treatment,TCM syndrome scores decreased(P<0.05),and the treatment group decreased significantly(P<0.01).(3)After treatment,there was no significant change in BMI or WHR,no statistical difference(P>0.05).(4)After treatment,FPG,2h PG,Hb A1 C and FINS o-f the two groups were all decreased(P<0.01),and there was a difference between thegroup s(P<0.05).(5)After treatment,homa-ir of both groups decreased before treatme-nt(P<0.01),and the decrease of homa-ir in the treatment group was more significant(P<0.01).(6)The level of TC,TG and ldl-c decreased after treatment(P<0.01),and the level of hdl-c increased(P<0.01).Compared with the control group,the treatment group was different(P<0.05).(7)After treatment,there was no significant change in ALT,AST and GGT levels,no statistical difference(P>0.05).(8)After treatment,the he patic b-ultrasonography of the treatment group was significantly improved compared with that before treatment(P <0.05),and the improvement of hepatic B hyperindex in the control group was not obvious(P>0.05),and the difference between the groups was statistically significant(P <0.05).(9)After treatment,serum Visfatin decreased significantly(P<0.01),and no significant change was observed in the control group(P>0.05).After treatment,RBP4 levels decreased(P<0.05),and serum SFRP5 increased(P<0.01).Compared with the control group,the level of serum Visfatin and RBP4 decreased significantly in the treatment group(P<0.05),and the level of SFRP5 increased significantly(P<0.01).(10)Safety analysis: no obvious adverse events occurred in the treatment group and the control group.Conclusion:Compound Gegenqinglian decoction can improve IRand correct inflamm atory state by regulating the expression of Visfatin,RBP4 and SFRP5,a related factor associated with T2 DM and NAFLD,so as to achieve the purpose of lowering glucose,regulating lipid,improving lipid metabolism,and observing the safety index of treatment group.It is proved that this prescription is safe and can be used in type 2 DM patients with NAFLD.
