BackgroundSince the liver is the site of most metastatic colorectal cancer,and liver metastasis is the most important cause of death of colorectal cancer.Therefore,how to prevent the occurrence of liver metastasis has become the direction of many clinicians.However,there is little research on extrahepatic metastasis.ObjectiveThis study was to investigate the clinicopathology risk factors of synchronous extrahepatic metastasis of colorectal cancer(CRC).MethodsA retrospective cross-sectional study was conducted to analyze 3763 consecutive cases of initial hospital admission(diagnosis)at the Affiliated Hospital of Qingdao University(3441 cases)and Shandong Provincial Tumor Hospital(322 cases)from 2010-01-01 to 2015-12-31.The complete disease data of patients with colorectal cancer included 12 risk factors that may affect extrahepatic metastases,namely,gender,age,hepatitis B surface antigen(HBsAg)positive,carcinoembryonic antigen(CEA),carbohydrate antigen 19-9(carbohydrate antigen 19-9,CA19-9),primary tumor long diameter,depth of primary tumor infiltration(T),regional lymph node metastasis(N),pathological type,location of primary tumor,degree of differentiation,and presence of liver Transfer.A single factor non-conditional logistic regression analysis yielded a meaningful index(P<0.05),which was a risk factor affecting extrahepatic metastasis.This risk factor was analyzed by multi-factor non-conditional logistic regression analysis to obtain a meaningful index(P<0.05),which is an independent risk factor for extrahepatic metastasis.Results3763 CRC patients,208 cases had synchronous extrahepatic metastasis with the prevalence of 5.53%.Logistic univariate analysis showed that:CEA,CA19-9,depth of invasion,regional lymph node metastasis,tumor location and liver metastasis were the risk factors of extrahepatic metastasis of CRC(all P<0.05).Logistic multivariate analysis proved that CA19-9,depth of invasion,regional lymph node metastasis and liver metastasis were independent risk factors for extrahepatic metastasis of CRC(all P<0.05).ConclusionCA19-9,depth of invasion,regional lymph node metastasis and liver metastasis are independent risk factors for extrahepatic metastasis of CRC.
