Objective To discuss the optimal energy of the monochromatic imaging with dual-source CT(DSCT)dual energy scan in reducing cochlear implantation electrode artifacts, and evaluate the value by HRCT after cochlear implantation.Methods(1)Collected the HRCT scanning data of 297 cases after cochlear implantation from May 2009 to December 2014,to evaluate the location, shape of cochlear implantation electrode,in order to study the imaging findings and its related factors.Selected 72 cases in all 297 cases, with MEDEL SONATA cochlear electrode implanted,36cases were scanned by GE Lightspeed 64 spiral CT,while the other 36 cases were scanned by dual-source CT,and monoenergetic software was used to post-process with the following 6 energies:40,70,100,130,160,190 KeV.Two experienced radiologists evaluated and graded the reformatted images with 6 different photon energies according 4-score scale respectively.Results:(1)In 297cases,286 cases of the electrode were implanted into cochlear completely with normal shape, while 1 case was implanted into horizontal semicircular canal,4 cases weren’t implanted completely with some electrodes exposeded outside of the.cochlear, which including 3 cases with inner ear malformation,6 cases of electrode were distorted discount.(2)There were statistical significance in abnormal implantation between with or without inner ear malformation group.(3)The score rank of 6 groups between 40to 190KeV were 19.50,55.86,95.98,166.57,159.64,153.85,with significantly statistical significance in the 6 groups. There were no statistically significant difference in the 3 groups of 130,160,190 KeV,but significantly statistical significance in 40,70,100,130 kev groups.(4)There were statistical significance in comparison with the imaging quality between 130 KeV by dual-source CT and GE Lightspeed 64 spiral CT.Conclusion:(1)Monoenergetic imaging of dual energy CT reduced the metallic artifacts for the cochlear implantation electrode, and improved image quality.(2)Images of 130 KeV showed the best image quality.(3)Images of 130 KeV by dual-source CT was better than GE Lightspeed 64 spiral CT.(4)HRCT was of important clinical significance in the post-operative assesment of cochlear implantation.
