| Objective: To explore the risk factors of the recurrence in differentiated thyroid carcinomaMethods: Including 560 cases of DTC in the First Affiliated Hospital of Guangxi Medical University from 2011 to 2017 were collected retrospectively.The diagnosis standard of recurrence of thyroid carcinoma according to surgical pathology.58 patients were diagnosed as recurrent thyroid carcinoma.The clinical pathological data includes the sex and age of patients,the type of pathological,greatest tumor diameter,single focal or multifocal tumor,thyroid capsular invasion,radioactive iodine treatment or not,thyroid neighbor tissue invasion,the number of neck lymph node metastasis.Analyzed the risk factors in Recurrent of differentiated thyroid carcinoma,and the correlation analysis including singular and multiple factors Was conducted with logistic egression method.Results:58 were recurrent in 560 DTC patients.Univariate analysis reveal that the maximum diameter of primary tumor(P=0.039),and cervical lymph node metastasis(P=0.003),lymph node metastasis of the central area(P=0.006),lymph node metastasis of lateral region of the neck(P=0.003),the operation type(P=0.049),TSH inhibition therapy associated(P=0.001)are relevant toDTC Recurrence.Age(P=0.778),Pathological type(P=0.092),Capsular invasion(P=0.312),Treatment of nail clearance(P=0.928)and gender(P=0.586)were not related to recurrence(see Table 1).Multivariate Logistic regression analysis showed that the maximum diameter of primary tumor >2cm(P=0.026,OR=1.667),TSH inhibition treatment(P=0.001,OR=0.093),cervical lymph node metastasis(P=0.001,OR=4.691)were related to the recurrence of DTC.Conclusion:The DTC patient with the diameter of tumor >2.0cm,cervical lymph node metastasis,and non TSH inhibition may be an independent risk factor for the recurrence of the patients with differentiated thyroid cancer.It is suggested that the recurrence of the patients with differentiated thyroid cancer is closely related to the above features. |
