Objective To investigate the detection value of Fluorescence in situ hybridization(FISH)technique and urine cytology in the diagnosis of urothelial carcinoma.Methods Urine specimens from 228 cases were collected in the first affiliated hospital of Soochow university from June 2016 to February 2018,which suspected urothelial tumor patients with hematuria.The FISH were used to measure the urine specimen chromosome 3,7,17 and 9 p21 locus anomalies,and traditional urine cytology examination was applicated to find tumor cells.Cystoscope,ureteroscopy and postoperative pathologic examination were the gold standard of urothelial tumors.At the same time,At the same time,20 normal people were selected as the control group.Results Among the 228 patients with hematuria,151 cases were non-cancer patients,and the result of one case was positive,the rest were negative by the FISH detection.All cases were negative with the urine cytology examination.77 cases were cancer patients,35 cases were positive,42 cases were negative by FISH;19 cases were positive,58 cases were negative by urine cytology examination,and 14 cases were positive by both FISH detection and urine cytology examination.Finally,the overall sensitivity of FISH in the diagnosis of urothelial neoplasm was higher than that in the urine cytology(45.45%,24.68%).The specificity of them were 99.34% and 100% respectively.The total sensitivity of the combined detection was 51.94% and the total specificity was 99.34%.Compared with individual FISH detection,the sensitivity of the joint method increased by 6.49% and the specificity did not change significantly.The sensitivity of the joint method increased by 27.26% compared with that of the urine cytology examination,and the specificity did not change significantly.The incidence of aneuploidy change and 9P21 deletion in chromosome 3,7 and 17 were 70.83%,58.33%,37.5% and 62.5%,respectively.There was no obvious correlation between the aberration rate of each chromosome and the stage and grade of bladder cancer.Conclusion The application value of FISH detection method in clinical diagnosis of urothelial carcinoma was higher than that of urine cytology examination,the combined detection has higher sensitivity,specificity,and noninvasive operation,high stability,and in the early diagnosis of urothelial carcinoma and postoperative follow-up is of great significance.
