| Objective:To search for some clinical factors about castration sensitivity(CSPC)progressing to castration resistant prostate cancer(CRPC)in the endocrine therapy of prostate cancer in this study.Materials and Methods:Collecting and sorting clinical data of 141 patients with prostate cancer giving endocrine therapy between January 2010 and December 2013 in Urology of the first affiliated hospital of Soochow University.58 patients had received radical therapy and adjuvant endocrine therapy(56 patients receiving radical prostatectomy,2 patients receiving radioactive iodine particles implantation).83 patients had received pure endocrine therapy.Single factor analysis and multiple factors logistic regression analysis were used to assess some factors about CSPC progressing to CRPC.Results:141 patients with prostate cancer have received androgen deprivation therapy(29 patients with bone metastases,112 patients without bone metastases).51 patients have progressed to CRPC.90 patients have not progressed to CRPC.Age,Cardiovascular history,PSA before endocrine therapy,Lowest value of PSA after endocrine therapy,T stage,Bone metastases in ECT,Time of PSA felling to lowest,Decline rate of PSA and Gleason score have correlated with time of progressing to CRPC by single factor analysis in the overall sample.Age,Time of PSA felling to lowest and Cardiovascular history have negatively correlated with progressing to CRPC.Gleason score,T stage,Bone metastases in ECT,PSA before endocrine therapy,Lowest value of PSA after endocrine therapy and Decline rate of PSA have positively correlated with progressing to CRPC.Age,Gleason score,Lowest value of PSA after endocrine therapy,Decline rate of PSA and Time of PSA fell to lowest have correlated with time of progressing to CRPC by single factor analysis in patients with radical therapy and adjuvant endocrine therapy.Age and Time of PSA felling to lowest have negatively correlated with progressing to CRPC.Gleason score and Decline rate of PSA have positively correlated with progressing to CRPC.Age,Cardiovascular history,Gleason score,PSA before endocrine therapy,Bone metastases in ECT,Lowest value of PSA after endocrine therapy,T stage,Time of PSA felling to lowest and Decline rate of PSA have correlated with time of progressing to CRPC by single factor analysis in patient with pure endocrine therapy.Age,Time of PSA felling to lowest and Cardiovascular history have negatively correlated with progressing to CRPC.Gleason score,T stage,Bone metastases in ECT,PSA before endocrine therapy,Lowest value of PSA after endocrine therapy and Decline rate of PSA have positively correlated with progressing to CRPC.Age,Lowest value of PSA after endocrine therapy and Decline rate of PSA have correlated with time of progressing to CRPC by Logistic regression in the overall sample.Age have negatively correlated with progressing to CRPC.Lowest value of PSA after endocrine therapy and Decline rate of PSA have positively correlated with progressing to CRPC.Lowest value of PSA after endocrine therapy and Decline rate of PSA have positively correlated with time of progressing to CRPC by Logistic regression in patients with radical therapy and adjuvant endocrine therapy.Lowest value of PSA after endocrine therapy and Time of PSA felling to lowest have correlated with time of progressing to CRPC by Logistic regression in patient with pure endocrine therapy.Lowest value of PSA after endocrine therapy have positively correlated with progressing to CRPC.Time of PSA felling to lowest have negatively correlated with progressing to CRPC.Area under the ROC curve(AUC)of Decline rate of PSA is biggest in all clinical factors in the overall sample.AUC of Decline rate of PSA is biggest in all clinical factors in patients with radical therapy and adjuvant endocrine therapy.AUC of Time of PSA felling to lowest is biggest in all clinical factors in patients with with pure endocrine therapy.Conclisions:In this study,Lowest value of PSA,Time of PSA fell to lowest,Gleason score,Decline rate of PSA are clinical factors of CSPC progressing to CRPC in all groups.Lowest value of PSA after endocrine therapy is clinical factor of CSPC progressing to CRPC by Logistic regression analysis in all groups.The lowest value of PSA lower,progressing to CRPC later after endocrine therapy. |
