Objective1.Initially explored the miRNA expression profile of CHF and provided new biomarkers for clinical diagnosis,treatment and prognosis assessment;2.Explored the correlation between Qi and Yin deficiency、blood and water stasis syndrome and Qi and Yang deficiency、blood and water stasis syndrome expression and miRNA expression spectrum,screen out the difference miRNA.And then provided the objective theoretical basis for the clinical syndrome differentiation of TCM in CHF;3.Based on miRNA preliminary explore the role of this method in the treatment of CHF.The miRNA with different expression after drug screening were used for target gene prediction and enrichment analysis,which provided a new target for the treatment of CHF with the principle of Yiqi Yangyin Huoxue Lishui,and provided a new direction and theoretical basis for the clinical and pharmacological research of traditional Chinese medicine.Methods1.Collected the CHF patients and CHF patients which belong to Qi and Yin deficiency、blood and water stasis syndrome and Qi and Yang deficiency、blood and water stasis syndrome,and healthy volunteers.Peripheral venous blood was collected and centrifuged to separate the plasma,and finally freeze it;2.Patients with Qi and Yin deficiency、blood and water stasis syndrome are randomly divided into treatment group and control group with 10 cases each.The control group was treated with routine western medicine.The treatment group is treated with Xinyin Tablets.After 8 weeks of treatment,the separated plasma was also frozen;3.All plasma was sent to Guangzhou Guanshan Limited company,total RNA was extracted and subjected to quality inspection.Then prepared the library and sequenced the miRNA according to the standard steps provided by Illumina.Finally,Target Scan and miRanda were used to performtarget gene prediction and enrichment analysis on the differentially expressed miRNA.Results1.Compared with the normal group,269 differentially expressed miRNAs were screened out in the CHF group(P<0.05),of which 34 were down-regulated and235 were up-regulated.There were 116 significantly differently expressed miRNAs(P<0.01),of which 109 miRNAs were up-regulated and 7 miRNAs were down-regulated.2.Compared with the normal group,154 miRNAs with differential expression were screened out in the syndrome of Qi and Yin deficiency、blood and water stasis(P<0.05),among which 117 miRNAs were up-regulated,and 37 miRNAs expression were down-regulated.And there were 49 miRNAs(P<0.01)with significantly different expressions,of which 38 miRNAs are up-regulated and11 miRNAs were down-regulated.Qi and Yang deficiency、blood and water stasis syndrome group was compared with the normal group.There were differentially expressed miRNAs(P<0.05),of which 140 miRNAs were up-regulated and 32 miRNAs were down-regulated.There were 52 miRNAs that were significantly differently expressed(P<0.01),of which 45 miRNAs were up-regulated,7 miRNAs expression were down-regulated.8 miRNAs with differentially expressed were screened out in comparison of Qi and Yin deficiency、blood and water stasis syndrome and Qi and Yang deficiency、blood and water stasis syndrome(P<0.05),among which4 miRNAs were up-regulated,the others down-regulated.3.After treatment,the miRNAs in the treatment group were compared with those before the treatment,and the miRNAs in the CHF group and the normal group were compared as subsets.The two subsets were then intersected.A total of 12 miRNAs related to CHF were treated with the method of Yiqi Yangyin Huoxue Lishui,among which 5 were up-regulated and 7 were down-regulated.Gene prediction and GO function analysis were performed on these 12 miRNAs,and a total of 985 target genes were screened.The main functions involve with transcription,DNA template,protein binding,signal transduction,small molecule metabolism,synthesis of cell nucleus and cytoplasm,adjust the ion channel and so on.Conclusion1.The presence of 269 differentially expressed miRNAs such as hsa-miR-34a-5p in the plasma of CHF patients might be a potential biomarker for diagnosis and prognosis assessment of CHF and a drug treatment target.2.154 miRNAs such as hsa-miR-21-5p could be used as diagnostic biomarkers forCHF as a syndrome of Qi and Yin deficiency、blood and water stasis.172 miRNAs such as hsa-miR-139-5p could be used as biomarkers for the diagnosis of CHF Qi and Yang deficiency、blood and water stasis syndrome.The 8 miRNAs such as hsa-miR-214-3p_R-1 could be used as the objective basis for distinguishing the two major types of syndromes: Qi and Yin deficiency、blood and water stasis syndrome and Qi and Yang deficiency、blood and water stasis syndrome.3.The mechanism of Yiqi Yangyin Huoxue Lishui method might involve the regulation of the expression of 12 miRNAs such as hsa-miR-27b-3p,which might affect the important life processes such as DNA template,protein and ATP binding.
