Objective:In this experiment,total hemoglobin concentration(HBT)and blood oxygen were measured before and after neoadjuvant chemotherapy(NAC)in patients with invasive ductal carcinoma of the breast by an ultrasound tomography image ultra-sonography system(OPTIMUS).The difference of saturation(S02)values,and the difference between the two groups according to different groups,to explore the clinical value of OPTIMUS in the evaluation of the efficacy of NAC in breast invasive ductal carcinoma.Methods:The subject of this experiment was the breast surgery visit of the First Affiliated Hospital of Kunming Medical University from January 2016 to December 2017.The puncture was confirmed as a breast infiltrating ductal carcinoma and 42 NAC patients were performed,and 42 patients were treated as The seventh edition of the AJCC guidelines TNM staging points T1,T2,T3,T4 group;RECIST1.1 standard points complete response(CR),partial remission(partial remission,PR),stable disease(SD),disease Progress(progressive disease(PD)group;pathological molecular classification LuminalA type,LuminalB type,human epidermal growth factor receptor-2(HER-2)overexpression type,triple negative breast cancer(TNBC),HER-2 positive VS HER-2 negative group.Using OPTIMUS to record the HBT and S02 values before and after NAC in each of these 42 patients(at least 4 times),and to analyze the differences in HBT and S02 values before and after NAC in different breast cancer subgroups,and to analyze their differences with invasive ductal breast cancer.T stage,breast cancer NAC efficacy,pathological molecular classification and other clinical features whether there is a correlation.The experimental data were statistically analyzed with SPSS22.0 statistical software.Measured data were expressed as mean±standard deviation.The t-tests were used among groups and within each group.P<0.05 was considered statistically significant.Results:1.Before NAC,6 cases were classified as Luminal A(14.3%),19 cases were Luminal B(45.2%),10 cases(23.8%)were LuminalB type HER-2 overexpression,and 7 cases were triple negative according to pathological molecular classification.16.7%),of which HER-2 positive patients were 19 cases(45.2%);according to lesion size:5 cases(11.9%)in T1 group,30 cases(71.5%)in T2 group,3 cases(7.1%)in T3 group,4 patients in the T4 group(9.5%);after NAC,grouped according to NAC effect:6 patients(14.3%)in the CR group,29(69.1)in the PR group,4(9.5%)in the SD group,and 3(7.1%)in the PD group.The effective rate of NAC is 83%.In patients with Luminal type A,the lesion size ranged from T3 to T4,and/or had breast-conservation requirements;T1 patients had severe lymph node metastasis(N3M0).2.According to the size of lesions,the P value of HBT change in T1 group(P=0.019)and the H value of HBT in T2 group(P<0.01)and S02(P<0.01)were all<0.05,which was statistically significant;3.According to NAC efficacy group:P group H group(P=0.002)change in P group and PR group HBT(P<0.01),S02(P=0.047)change P value<0.05,with statistical significance;4.According to the breast cancer pathological molecular classification group,the P value of HBT changes in all groups was<0.05,with statistical significance,and the P value of LuminalB(HER-2 negative)type S02 changes was<0.05,which was statistically significant;-2 positive and negative groups compared HACT,SO2 before and after NAC,P value<0.05,with statistical significance;5.According to the breast cancer pathological molecular classification group,there was a statistically significant difference in HBT values before and after comparison of NAC between groups(P=0.024<0.05);6.According to the lesion size and NAC efficacy group,there was no significant difference in HBT values before and after comparing NAC between groups(P>0.05);there was no significant difference in SO2 value between groups before and after NAC(P>0.05).Conclusions:1.Changes in HBT values of ultrasound light scattering imaging system before and after NAC lesion size,NACThe curative effect was positively correlated and could be used as an indicator of breast cancer NAC efficacy.2.The change of the parameter SO2 value of the ultrasound light scattering imaging system is positively correlated with the size of the lesions before and after the NAC and the partial grouping of the NAC efficacy.The SO2 can be used as a secondary reference index for evaluating the efficacy of the breast cancer.3.According to the lesion size,pathological molecular classification and NAC efficacy of breast cancer,there was no significant correlation between HBT and S02 values before and after NAC in each group.
