Mental Health Literacy:Survey Of First Year Medical Postgraduates Among Three Medical Universities In Guangzhou

Objectives:To assess level of Mental Health Literacy(MHL)among 714 medical postgraduates in 3 Guangzhou medical universities.Specific objectives were included:1.To investigate medical postgraduates’ attitude toward seeking professional mental help and its influencing factors.2.To investigate medical postgraduates’ attitude toward mental ill patient and its influencing factors.3.To investigate the level of basic mental health knowledge among medical postgraduate and its influencing factors.4.To investigate medical postgraduates’ knowledge of 4 common mental disorders and its influencing factors.Methods:In this study,714 medical postgraduates from 3 medical universities were recruited via the combination of stratified sampling method and cluster sampling method.Inventories were applied to investigate students’ MHL,including Mental health knowledge questionnaire(MHKQ),Vignette tests,Kessler-10 inventory(K-10),Attitudes toward seeking professional psychological help-short form(ATSPPH-SF)and scale of personal attitude toward mental ill patient.Results:1.Research of students’ attitude toward seeking professional psychological help1.1 Score of attitude toward seeking professional psychological help(ATSPPH)was(31.62 ± 3.83),3.8% of the student got score lower than 20.1.2 Results showed students’ ATSPPH score were influenced by several factors,including students’ specialties,arrangement of psychiatry or psychology curriculum,working experience,mental health status,usage of mental health service,interest toward mental health,mental health info acquisition,sources of getting mental health info and source of getting help(P<0.05).Finally,Multiple Linear Regression identified 3 factors have an impact on students’ MHKQ score,including mental status(B=-1.373,P=0.000),ways of getting mental health info(B=1.449,P=0.000),usage of mental health service(B=-0.942,P=0.006).Above factors explained 10.6% variance of the equation.2.Research of students’ attitude toward mental ill patient2.1 Score of attitude toward mental ill patient was(38.28 ± 4.59),5.0% of the student got score lower than 30.2.2 The overal score of attitude toward mental ill patient was influenced by below factors,including students’ specialties,arrangement of psychiatry or psychology curriculum,gender,education level,mental health status,previous contact with mental ill patient,interest toward mental health and sources of getting mental health info(P<0.05).Finally,Multiple Linear Regression identified 4 factors have an impact on students’ MHKQ score,including mental status(B=-2.160,P=0.000),gender(B=1.179,P=0.001),previous contact with mental ill patient(B=1.378,P=0.000),interest on mental health(B=1.480,P=0.000).Above factors explained 9.3% variance of the equation.3.Research of basicl mental health knowledge3.1 The MHKQ score of medical postgraduates were(16.41 ± 2.68).76.3% of student got score equal or higher than 16,met the basic requirement set by the national health administration.In the MHKQ questionnaire,some items’ correct rate lower than 80%,including memorial date of mental health(28.9%~65.3%),causes of mental disorders(75.3%~76.4%),prevention of mental disorders(70.4%~78.9%).3.2 Results showed students’ MHKQ score were influenced by below factors,including students’ specialties,gender,working experience,usage of mental health service,arrangement of psychiatry or psychology curriculum,interest toward mental health,mental health info acquisition and sources of getting mental health info(P<0.05).Finally,Multiple Linear Regression identified 5 factors have an impact on students’ MHKQ score,including mental status(B=-2.376,P=0.000),gender(B=0.500,P=0.004),interested on mental health(B=1.443,P=0.000),ways of getting mental health info(B=0.690,P=0.003),and arrangement of psychiatry or psychology curriculum(B=0.692,P=0.002).Above factors explained 27.2% variance of the equation.4.Knowledge of 4 common mental disorders4.1 The overall score of vignette-test of medical postgraduate was(15.22 ± 3.42),student score(3.38 ± 1.17)at depression vignette,(4.28 ± 1.07)at schizophrenia vignette,(3.66 ± 1.35)at General anxiety disorder vignette(GAD)and(3.91 ± 1.38)at phobia vignette.4.2 Details about students’ beliefs and knowledge of 4 common mental disorders4.2.1 Result showed students’ recognition rate of schizophrenia,major depressive disorder(MDD),phobia and GAD vignette was 81.2%,65.9%,46.1% and 35.9%,respectively.4.2.2 Student believed that social-psychological factor(30.9%~68.8%)and person’s character(23.2%~56.2%)were the major cause of 4 mental disorders.Some students(4.8%~15.7%)believed biological factors could be the cause of 4 mental disorders.Finally,very few students(0.7~3.6%)believed that superstition issues(such as bad fengshui,evil curse)were the cause of 4 mental disorders.4.2.3 Result showed 84.9%,51.7%,50.6% and 48.7% student recommended professional help or intervention to the patient in schizophrenia vignette,GAD vignette,MDD vignette and phobia vignette.What’s more,many students believe that non-professional help or intervention,such as chat with other,take some rest,could combat depression(42.3%),GAD(40.1%)and phobia attack(28.2%),solely and effectively.4.2.4 Student believed patient in 4 vignettes would rather be odd: 25.8%~53.6% or danger: 31.7%~56.2%.4.2.5 About 93.4% and 81.9% of student believed that patient with schizophrenia or depression could work as usual unless they were treated.However,few students hold same view to patient with GAD(50.4%)or phobia(61.5%).High portion of them believed GAD(47.9%)or phobia(38.2%)patient just need some rest.4.3 Non-parametric test reviewed that vignette-test score was influenced by below factors,including students’ specialized,arrangement of psychiatry curriculum,interested on mental health,and mental health info acquisition.Finally,Multiple Linear Regression showed 4 factors have impact on overall vignette-test score,including mental status(B=-0.683,P=0.010),interest on mental health(B=3.255,P=0.000)and arrangement of psychiatry or psychology curriculum(B=1.284,P=0.000).Above factors explained 26.8% variance of the equation.Conclusions:1.Medical postgraduate showed positive attitude toward seeking professional psychological help.Factors that influenced students’ attitude were their mental status,status of getting mental health info and usage of professional mental health service2.Medical postgraduate showed positive attitude toward mental ill patient.Factors that influenced students’ attitude were interest on mental health,previous contact with mental ill patient,gender and mental health status.3.Medical postgraduate had high level of basic mental health knowledge.Factors that influenced students’ attitude were interest on mental health,arrangement of psychiatry or psychology courses,mental health status and source of getting help.4.Medical postgraduate had insufficient knowledge of phobia,general anxiety disorder and depression.Factors that influenced students’ knowledge of mental disorders were interest,their mental status and arrangement of psychiatry or psychology courses.5.The overal mental health literacy of medical postgraduate was good,but with shortcoming.Medical postgraduates’ mental health literacy was mainly influenced by 3 factors,personal intrest on mental health,arrangement of psychiatry or psychology curriculum and their mental health status.Future intervention targeting on medical postgraduates’ mental health literacy should aware the impact of above factors.
