Objectives: To explore the related factors of the occurrence of white matter lesions,and to explore the relationship between the serum levels of cystatin C and the severity of white matter lesions,and to provide evidence for the prevention and treatment of white matter lesions and their severity prediction.Methods: A total of 214 consecutive cases of leukoencephalopathy diagnosed by head magnetic resonance imaging were collected from October 2016 to March 2107 in neurology department of our hospital,including 130 males and 84 females,aged 50-85 years.Average 63.86 ± 8.735 years old.Head magnetic resonance imaging(MRI)was used to evaluate the white matter lesions.According to the lesion site,the white matter lesions were divided into P-WMLs and D-WMLs.According to the Fazekas scale,the severity of cerebral ventricular white matter lesions and subcortical deep white matter lesions were scored and divided into no-mild group,moderate-severe group according to the scores.General information(name,age,sex,hospital number)and past history(history of risk factors for cerebrovascular diseases such as hypertension,diabetes,smoking,alcohol consumption)were collected.All patients were collected fasting venous blood within 24 hours after admission and the triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterin(LDL)),Serum creatinine(CREA),serum cystatin C(Cyst-C),hs-C reactive protein(hs-CRP)and homocysteine(Hcy)Determination of related indicators.All data are processed using SPSS19.0 statistical software.The measurement data subject to normal distribution is expressed as(?x ± s).If the variance homogeneity is satisfied,the two independent t-tests are used.If the variance is not homogeneous,the Wilcoxon rank-sum test of two independent samples is used.Non-normal distribution of measurement data,using the median / quartile said that the comparison between groups using rank sum test.Counting data using χ2 test.Analysis of risk factors of white matter lesions using Logistic regression analysis.Statistical significance of the indicators included in the Logistic regression,P <0.05 was considered statistically significant.Result: Patients with general data were compared.Univariate analysis of ventricular white matter lesions,two groups of patients with gender composition,history of diabetes,smoking history,Scr no significant difference.Patients in the moderate-severe group had a higher prevalence of hypertension,lower TC levels,and higher levels of hs-CRP,Cyst-C,and Hcy than those in the mild-PML group The difference was statistically significant(P <0.05).Univariate analysis of deep white matter lesions under the cortex revealed no significant difference in gender composition,diabetes history,smoking history,TG,TC and CRE between the two groups.The patients in the moderate-severe group were older,the prevalence of hypertension was higher,the levels of Cyst-C,Hcy and hs-CRP were higher than those in the mild group without D-WMLs,and the differences between the two groups were statistically significant(P <0.05).Logistic regression analysis of influencing factors of white matter lesions In P-WMLs patients,P-WMLs was taken as dependent variable(no-mild degree)with age,hypertension,TC,Cyst-C,Hcy and hs-CRP as independent variables Group assignment was 0 and medium-severe group assignment was 1).Logistic regression model was established.The results showed that age,hypertension and Cyst-C were independent risk factors of P-WMLs.D-WMLs patients,age,hypertension,Cyst-C,Hcy,hs-CRP as independent variables,D-WMLs different groups as dependent variables(no-mild group assignment is 0,),Two-class Logist regression analysis showed that age,hypertension and Cyst-C were independent risk factors for D-WMLs.Conclusion: Age,hypertension,serum cystatin C is an independent risk factor for left ventricular white matter lesions and subcortical deep white matter lesions.Serum Cystatin C levels reflect the severity of white matter lesions.
