Community acquired pneumonia is one of the most common infectious diseases seriously endangering human health,and its mortality is the the leading cause of infectious diseases.In recent years,despite the use of powerful antibiotics and effective vaccines,the mortality rate of pneumonia is still high and even rising.There are a wide variety of pathogens in community acquired pneumonia.In the past,Streptococcus pneumoniae was considered as the main pathogenic pathogen.However,in recent years,more and more pneumonia patients were diagnosed with atypical pathogens,with the continuous improvement of diagnosis and treatment methods.Among the patients with atypical pathogens,the infection rate of Mycoplasma pneumoniae was the highest.The pathogenesis of Mycoplasma pneumoniae is complicated,and it is not very clear at present.Some scholars have proposed that it can cause inflammation and tissue damage through the mechanisms of Mycoplasma pneumoniae adhesion,invasion,direct cell injury and immune inflammation.Early detection of Mycoplasma pneumoniae is of great importance to clinical treatment.There are many methods of detection at present,including isolation and culture,gene diagnosis,serological detection,etc.Serological specific IgM antibodies are widely used in clinic because of their short time,simple operation,high sensitivity and specificity.Objective:To analyze the changes of PCT,CRP and D-dimer,FIB,PT,APTT,AT-III and other related indicators in patients with Mycoplasma pneumoniae IgM positive pneumonia.To explore the similarities and differences in clinical manifestations,inflammation,coagulation and fibrinolysis between Mycoplasma IgM antibody positive and negative pneumonia.Method:A total of 234 cases of community-acquired pneumonia were selected from the respiratory department of the affiliated hospital of chengde medical college from January 2017 to December 2017.According to whether the IgM antibody of Mycoplasma pneumoniae was positive,the patients were divided into IgM positive group(79 cases)and Mycoplasma pneumoniae IgM antibody negative group(155 cases).Clinical and laboratory data,imaging and other related data were collected,and then the differences between the two groups were observed.Results:1 The comparison of the general data between IgM antibody positive group and IgM antibody negative group:There was no significant difference in the sex and age between the IgM positive group and the negative group(P > 0.05).2 The comparison between IgM antibody positive group and IgM antibody negative group in the clinical manifestation,the severity of the disease and the imaging:In the clinical manifestations,the two groups of patients showed fever,cough,dyspnea and so on,and the difference was not statistically significant(P > 0.05).In the severity of the disease,the positive rate of bacterial culture in the IgM antibody positive group was 23.6% higher than that in the IgM antibody negative group(12.3%);In the CURB-65 score,the score of 2 to 5 points was 32.9% in the IgM positive group,and 18.1% in the IgM negative group;The incidence of severe pneumonia in the IgM antibody positive group was 20.3% higher than that of the IgM antibody negative group(9%),and the above indexes were statistically significant in the two groups(P < 0.05).In imaging findings,more performance for multiple lung infiltrates in both groups,in the IgM antibody positive group(54.3%),IgM antibody negative group(56.8%),is similar between the two groups(P > 0.05).3 The comparison between the IgM antibody positive group and the IgM antibody negative group was PLT,PCT and CRP:The PLT count of the IgM antibody positive group was lower than the IgM antibody negative group,while the inflammatory marker PCT and CRP were all higher than the IgM antibody negative group(P < 0.05).4 The comparison between IgM antibody positive group and IgM antibody negative group was D-Dimer,FIB,PT,APTT and AT-III:The values of D-Dimer and FIB of IgM antibody positive were higher than IgM antibody negative,while the values of PT,APTT and AT-III were lower in IgM positive group(P < 0.05).Conclusions:1 There is no difference in general clinical manifestations and imaging findings between pneumonia positive and negative pneumonia patients with IgM antibody.2 Mycoplasma pneumoniae IgM antibody positive pneumonia patients with severe pneumonia incidence and CURB-65 score is high.3 Mycoplasma pneumoniae IgM antibody positive pneumonia patients had more severe inflammatory reaction and more severe disorder of coagulation and fibrinolysis system.
