Objective: To investigate the source of negative emotion and its maladaptive cognition in caregivers who provide caregiving in nursing homes,following by the investigation of the effectiveness of cognitive reappraisal group counseling to decrease negative experiences of anxiety and depression in caregivers of whom are physically impaired in old age.Method: 1.Thirteen caregivers were selected to have a personal-in-depth interview through semi-structural qualitative researching method and the collecting materials of caregivers' perception to stimulating events,emotional experience and their favored pressure relief approached were processed and encoded by Colaizzi's 7-step analysis.2.Sixty-one caregivers of physically impaired elders were allocated into experimental group(n=31)and control group(n=30)which receiving cognitive reappraisal group counseling and general support separately by stratified-randomization.The data which were collected by SAS,SDS,ERQ-CRV and a group member self-assessing questionnaire were analyzed to compare the effectiveness on anxiety,depression experience and emotion regulation strategy.Results: 1.Three themes were extracted from the sources: a.Difficulties in caregiving;b.Vocational gains inadequacy;c.Supporting insufficiency.Two themes were extracted in mitigation: a.Cognitive adjusting;b.Emotions letting-out.2.After 8 weeks of intervention,the SAS,SDS and ERQ-CRV scores in the experimental group were promoted and significantly different from the control group.The changing of ERQ-CRV score in experimental group shows that their emotion regulation strategy has been improved.The before-and-after score variations in experimental group are significantly higher between two groups in any outcome.Conclusions: Caregiving in nursing homes as an occupation to nursing assistants can lead to special emotional experiences comparing with family caregivers,however,there were far more studies on family caregivers burden and emotional experience than formal ones.It maintains a conscious deficiency and ignorance of psychological well-being,coping with negative emotions in nursing assistants themselves.As co-workers and supervisors,nurses have the duty and ability to provide psychological nursing or some necessary actions in reliving their mental stress and try the best to offer a wholesomely supportive environment of working.In this study,the results indicate that cognitive reappraisal strategy can decrease caregivers' anxiety and depression experiences from working scenes and improve their emotion regulation strategy.The study helps us know better about cognitive influences in emotion regulation and it also gives us some clues in psychological nursing approach to persons with negative emotion experiences.
