| Research background:Heart failure(HF)is a kind of for various reasons caused by heart disease eventually led to the clinical syndrome,cardiac insufficiency is the most serious stage of heart disease,it is becoming a 21st century the most important cardiovascular disease[1].Diastolic heart failure[2]is to point to a few cases of myocardial contraction force can be made to maintain normal cardiac output,but due to the increased abnormal left ventricular filling pressure,make the pulmonary vein circumfluence suffocate suffocate,and cause pulmonary blood clot.Homocysteine(HCY)also called homocysteine,is one of the types of the body containing sulfonium non-essential amino acids,but the body itself can not synthesis,only the body after eating food containing methionine,methionine demethylation effect through transformation.Experimental research data show that high homocysteine levels,promoting the possible mechanism of the occurrence and development of atherosclerosis,is through the destruction of endothelial cell function and stimulating platelet-derived growth factor effect on promoting cell division,at the same time involved in the formation of blood clots[3].In recent years,with domestic and foreign scholars gradually deepening research of HCY,found in patients with chronic heart failure of HCY in plasma mass concentration increased significantly,speculated that it might be a lead to a new risk factor for heart failure[4].But,for the development of related research situation at home and abroad,about the quality of HCY concentration and heart failure correlation research,and on it with diastolic heart failure related to the severity of cardiac function research less.Objective:To observe the diastolic heart failure(DHF)patients with plasma homocysteine and N-terminal brain natriuretic peptide-concentration(NT-proBNP),and explore high homocysteine(HHCY)and DHF and severity(including cardiac function,LVEDD,LVEF,NT-proBNP)relationship.Methods:Diagnosis of January 2014 to January 2015 I received medical treatment for patients department diastolic heart failure 60 cases and 40 cases of patients with cardiac decompensation to diastolic heart failure group and cardiac decompensation group,at the same time It included 40 cases of healthy people as a control group.All patients were in the hospital the next morning fasting blood 5ml measuring HCY,NT-proBNP and other biochemical markers,hospital cardiac B-ultrasound recording of left ventricular end-diastolic the inner diameter and left ventricular ejection fraction.Results:(1)Three groups in general information have no statistical significance(P>0.05).(2)in the three groups,patients with diastolic heart failure group of HCY blood concentration of 53(36.25,66.00)higher than the heart function compensatory group 15.35±2.48,both are higher than that of normal control group 8(7.00,10.00),comparing differences between groups was statistically significant(p<0.05).And three groups,Compared with LVEDD,LVEF,NT-pr oBNP difference was statistically significant(p<0.05).(3)in diastolic heart failure group,with the increase of NYHA heart function classification,plasma HCY inpatients increased levels of the NT-proBNP and LVEDD increases gradual ly,LVEF gradually reduce,at all levels between patients HCY,NT-proBNP,an d LVEDD difference was statistically significant;Compared with cardiac functi on III group,LVEF difference has no statistical significance,the difference w as statistically significant compared with the cardiac function IV group.(4)The correlation analysis between HCY and NT-proBNP in patients with diastolic heart failure showed that HCY was positively correlated with NT-proBNP.Conclusions:(1)Diastolic heart failure in patients with plasma HCY levels higher than that of patients with cardiac compensatory group,both are higher than that of normal control group,shows that HCY is associated with heart failure,may be one of the independent risk factors for the development of diastolic heart failure.(2)Diastolic heart failure in patients with plasma HCY,NT-proBNP,LVEDD levels increased with the increase of cardiac function classification,the level of LVEF and decreased with the increase of cardiac function classification,and HCY as BNP,LVEDD rises,and decreased with the increase of LVEF,show that HCY concentration can response the severity of heart failure,can indirectly reaction in clinical work change heart structure and function in patients with diastolic heart failure damage. |
