| Objectives:This study foucused on 1)the prevalence of health-related quality of life of pregnant women in six districts of tianjin;2)risk factors for low health-related quality of life of pregnant women in six districts of Tianjin;3)effect of low quality of life on gestational diabetes in pregnant women in six districts of Tianjin. Methods:From December 13th,2010 to October 11th,2012,more 20,00 pregnant women were registered at primary health service center in the six districts(Heping District,Hebei District,Hexi District,Hedong District,Hongqiao District,Nankai District)of Tianjin,China.According to the criteria of inclusion and exclusion, the cases of pregnant women eventually were 10,943.All pregnant women participated in the screening, and a questionnaire of“Health questionnaire during early pregnancy”was filled out, which including pregnant women’s basic demographic characteristics and anthropometry data. All pregnant women were registered within the first 32gestational weeks at primary health service centers, and were then managed in 25secondary or tertiary hospitals. At primary health service centers, all pregnant women were routinely offered a non-fasting 50-g 1-h glucose challenge test between 24th and28th weeks of gestation.Women with positive glucose challenge test,i.e.,≥7.8mmol/L,were referred to a centralized GDM clinic located within Tianjin Women and Children’s HealthCenter, where they underwent a standard 75-g 2-h oral glucose tolerance test(OGTT).Gestational Diabetes Mellitus(GDM)was diagnosed according to the IADPSG diagnostic criteria. At the same time, all pregnant women filled in the“SF-36”and“Mid-term pregnancy health questionnaire”,which including basic demographic characteristics and anthropometry data.Binary logistic regression was used to obtain odds rations(ORs)and 95%confidence intervals(CIs)of influence factors of low health-related quality of life of pregnant women.Using age, quality of sleep, life pressure, and education level as independent variables, the eight domains scores and two summary scores of quality of life were used as dependent variables,to explore the impact of quality of life on pregnant women in six districts of Tianjin.Taking each domains of quality of life as an independent variable and gestational diabetes as the dependent variable,to explore the effect of low quality of life on gestational diabetes. Results:In the eight domains and two aspects of scores of quality of life of pregnant women in the six districts of Tianjin, the median of the physical role function was the lowest, which was 50.The score of physical health(PCS)was second, and the medain is 68.Score of other aspects is over 70 points, and some even reach 80-100 points.Sleep quality is a significant factor, and it is associated with every domains of quality of life.Life pressure affects seven domains of quality of life, and age affects six domains of quality of life.Other factors, such as education level, monthly income level, drinking, smoking, etc.affect many aspects of quality of life.Quality of life(QOL)was no difference between women with gestational diabetes and those without gestational diabetes.About the impact of low quality of life on gestational diabetes in pregnant women, the study found that the general health(GH)of pregnant women is an influencing factor of gestational diabetes. Conclusion:The overall quality of life of pregnant women in the six districts of Tianjin is good, but the physical role(RP)was poor and the score was the lowest.Sleep quality, life stress, age, and vomiting during pregnancy are the four primary factors that have a significant impact on the quality of life of pregnant women.Others, such as education level, monthly income, gestational weeks, smoking and drinking, have different effects on various areas of the quality of life of pregnant women. The quality of life is no difference in between gestational diabetes patients and non-patients;Low general health of QOL may be an influencing factor for gestational diabetes. |
