Study On Clinical Features Of Hypertensive Disorder Complicating Twin Pregnancy

Objective:Hypertensive disorder during pregnancy is one of the most common gestational diseases that threaten maternal and child health.Twin pregnancy is a high risk factor for hypertensive disorder complicating pregnancy,which aggravates the condition of hypertensive disease during pregnancy.With the promotion of assisted reproductive technology,the incidence of twin pregnancies has increased year by year.The incidence of hypertensive diseases in twin pregnancies has also gradually increased,further jeopardizing the lives of mother and children.To better understand the risk factors and the severity of pregnancy of hypertensive disorders between twin versus single pregnancy of hypertensive disorders as well as normal pressure twin pregnancy,we analyze those patients clinical data.Methods:Retrospective analysis of the maternal deliveries of hospital delivery at the General Hospital of Tianjin Medical University in January 2016-September 2017.The twin-pregnant patient with hypertensive disorder was selected as the study group.Patients with a single child and hypertensive disorder complicating pregnancy were included as a control group 1.Normal twin pregnancy was defined as another control group 2.Comparing the general conditions,complications,and pregnancy outcome between those groups.Results: 1.The incidence of gestational hypertensive disorders(30.4%)was higher in twin pregnancy during the 2016-2017 period than in singleton pregnancy(12.5%).The difference was statistically significant(P<0.05).2.The incidence of preeclampsia-eclampsia in the research group was higher(64.9%)than in the control group 1(50.3%).The difference was statistically significant(P<0.05).3.The gestational week of delivery in the research group were less than the control group 1,and the number of days of hospitalization was greater than that of the control group 1,the difference was statistically significant(P<0.05).The number of days of hospitalization in case group was greater than control group 2,and the number of gestational weeks was less than control group 2,the difference was statistically significant(P<0.05).4.The rate of primigravida,the ages of pregnant women ≥35,IVF-ET technology were higher in research group than in control group 1.The difference was statistically significant(P<0.05).The rate of IVF-ET in research group was higher than that in control group 2,the difference was statistically significant(P<0.05).5.The incidence of hypoproteinemia,cardiac insufficiency,anemia,severe postpartum hemorrhage and severe edema were higher in research group than in control group1(P<0.05).The incidence of hypoalbuminemia,cardiac insufficiency,anemia and severe edema was higher in the research group than in the control group 2(P<0.05).6.The rate of low birth weight in research group were lower than in control group 1.The cesarean section rate,preterm birth rate,and low birth weight infants were higher than the control group 1.The difference was statistically significant(P< 0.05).The low birth weight,ICU treatment rate,cesarean section rate,preterm birth rate,and incidence of low birth weight infants were higher than those of control group 2,the difference was statistically significant(P<0.05).Conclusion: The twin pregnancy had a high prevalence of hypertensive disorders during pregnancy.The twin-pregnant patient with hypertensive disorder had a higher proportion of preeclampsia-eclampsia,especially in severe preeclampsia.The ages of pregnant women ≥35,IVF-ET,and primigravida are risk factors for twin pregnancy with hypertensive disorders of pregnancy,and had higher gestational weeks of delivery and low neonatal birth weight.Research group also had higher rates of heart failure,severe edema,hypoproteinemia,and severe postpartum hemorrhage.When twin pregnancies have severe conditions such as hypertensive disorders during pregnancy,they should be actively treated to prevent maternal and child complications.
