The evaluation of oral motor function can reflect the flexibility and accuracy of the movement of mandibular,lip,tongue and other organs,so it is widely used as one of the most important and effective measures to evaluate the speech ability of the patients with dysarthria.Cleft lip and palate,as a typical representative of organic dysarthria,found that most of the children with cleft lip and palate have different degrees of articulation disorder,especially for children with velopharyngeal insufficiency after cleft lip and palate repair,speech disorders are more obvious.And in the study of the clinical speech rehabilitation of cleft palate patients,it is found that there are structural and functional abnormalities in the jaw,lip and tongue of the cleft palate patients,and foreign studies have found that good velopharyngeal closure is not the only condition for common speech,and the coordinated movement of mandibular,lip,tongue and teeth is also one of the most important conditions for normal speech.But the movement function of the mandibular,lip,tongue and other organs has the specific features,and there is no further study on it.And in the clinical speech rehabilitation training,we can also find that the therapist pays more attention to the speech intelligibility of the cleft palate patients and often neglects the movement ability of the jaw,lip,tongue and other organs of the cleft palate patients,which is the focus of this study.Therefore,this study intends to select the children of cleft palate as the research object,and to explore the characteristics of the verbal and non verbal state of the oral motor ablity and the correlation with the speech articulation.The main contents and results of this study are mainly divided into the following parts.1.A study on the characteristics of oral sensory-motor ability of the cleft palate children.In this study,we evaluated and analyzed the oral sensory-motor ability of children with Ordinary,VPA cleft palate,VPI cleft palate,the results of the study are as follows:(1)Age: there is a significant age difference in the overall development of the oral sensory motor ability of the three types of children,showing that the oral movement ability of the mouth is more and more mature as the age increases;and the mouth sensation development of normal children and VPA cleft palate children is mature at the age of 3;but VPI cleft palate children The sensory development of the mouth is relatively slow,especially at the age of 3 and 4 years of VPI cleft palate children still have the problem of the mouth sensation,but at the age of 5 they also tend to develop.The tongue movement was significantly different.(2)Sex has a significant effect on the oral sensory-motor ability of normal children,especially the movement of the lower jaw and tongue,but no significant effect on children with cleft palate.(3)The degree of velopharyngeal closure has an extremely significant effect on the sensory and motor ability of the mouth of three types of children,and the palatine palatopalatine children especially with VPI cleft palate have the worst sensory motor ability,and the order of sensory motor ability from high to low in the mouth is the same as common children > VPA cleft palate children > VPI cleft palate children.2.A Study on the characteristics of children with cleft palate diadochokinesia abilityIn this study,we evaluated and analyzed the diadochokinesia ability of children with normal,VPA cleft palate,VPI cleft palate,the results of the study are as follows:(1)There are very significant or extremely significant differences in the parameters of diadochokinesia ability in three types of children with different velopharyngeal closure.(2)In terms of different corpus,diadochokinesia rate,speech rate,articulation rate in the normal children and the two types of cleft palate are all decreasing with the increase of the number of syllables;The overall trend of the average syllable duration,average pause duration is increasing as the number of syllable increases;the pronunciation duration ratio,accuracy are all decreasing with the increase of the number of syllables.As the number of syllables increased,the overall performance of diadochokinesia ability gradually became worse,but the performance from high to low was still: common children > VPA cleft palate children > VPI cleft palate children.3.Relationship between oral sensory-motor ability and speech clarity in children with cleft palate.In this study,the correlation between oral sensory-motor ability and speech clarity in children with VPI cleft palate,VPA cleft palate children and normal children was analyzed.(1)The oral sensory-motor ability of children with VPA cleft palate is significantly correlated with sentence articulation under continuous speech.(2)The oral sensory-motor ability of children with VPI cleft palate was not correlated with all indicators of speech articulation.To sum up,the sensory motor ability of normal children and VPA cleft palate children is related to the sentence clarity under the continuous flow,while the sensory motor ability of the VPI cleft palate children is not related to the indexes of speech articulation.Conclusion: in the speech rehabilitation training for VPA cleft palate children,we should not only pay attention to the definition of single word,but also pay attention to the articulation of the sentence in continuous flow,and improve the ability of synergetic tone better through the exercise of the sensory exercise ability of the mouth,so as to improve the continuous voice clarity of the VPA cleft palate children.Degree.However,for VPI cleft palate children,the repair of velopharyngeal closure function is the precondition for speech training.On this basis,proper oral exercise training can improve the speech ability of the palatopalatine children.4.Study on the correlation of children with cleft palate diadochokinesia ability and clarity clarity.In this study,the correlation between VPI and VPA children and normal children diadochokinesia ability and speech clarity are analyzed:(1)The diadochokinesia rate,and the average syllable duration of the VPI cleft palate children were significantly related to the definition of the vowel word,while for the difficult double syllable corpus /taka/,the speech rate,articulation rate were significantly related to the definition of the vowel and the definition of the vowel sentence.(2)The average pause duration,pronunciation duration ratio,accuracy of VPA cleft palate children were mainly related to speech intelligibility,such as monosyllabic /ka/,double syllable /paka/,/taka/ and three syllable /pataka/ with tongue root sound.(3)The diadochokinesia rate,speech rate,articulation rate,average syllable duration,average pause duration,pronunciation duration ratio,accuracy rate of normal children are mainly related to the articulation of monosyllabic /ka/ with tongue /k/,double syllable /pata/,/paka/,/taka/,and three syllable /pataka/,The normal children situation is similar to VPA cleft palate,but there is no correlation between the average pause duration of pause and indicators of speech articulation.Conclusion: two types of cleft palate children,VPI and VPA,have problems such as slow speech speed,easy pause and low accuracy in diadochokinesia fluency aspect.In speech training,we should strengthen the ability of harmonization of articulatory organs to improve the accuracy and fluency of speech.In addition to paying attention to the consonant exercises of the consonants,it is necessary to strengthen the practice of the vowel in the articulation of the sound,especially in the pronunciation practice of the tongue,to strengthen the single word,word,sentence structure with the tongue root sound /k/.Sound exercises.
