Objective(1)The purpose of this paper was to test muscle mass,grip strength,gait speed,appendicular skeletal muscle mass index and other related indexes of sarcopenia in the elderly,to discuss the prevalence of sarcopenia in people with Osteopenia/ Osteoporosis in Zhenjiang area,and to compare the difference in the prevalence of sarcopenia among osteoporosis,osteopenia and normal groups.(2)Further to explore the association between sarcopenia and osteopenia/osteoporosis in Chinese elderly and estimated the risk of developing sarcopenia among Chinese elderly with osteopenia/osteoporosis.MethodsA total of 191 subjects were analysed.All subjects were more than or equal to 60 years old.Height and weight were measured.Body mass index(BMI)was calculated as body weight in kilograms divided by the square of body height in meters.For BIA,whole body composition was analyzed by a multi-frequency bioelectrical impedance analysis device(MC-180,TanitaCo,Ltd,Japan),finally the skeletal muscle index(SMI)was calculated as ASM divided by the square of body height in meters.Muscle strength was assessed by handgrip strength.Physical performance was examined by 4-meter gait speed with each participant.Bone densitometry by dual-energy X-ray absorptiometry(DXA)was performed using the equipment Lunar DPX NT(GE Medical Systems,Madison,WI USA).All subjects were divided into Osteoporosis group,Osteopenia group and Normal group according to the definition of WHO(World Health Organization).According to the diagnostic criteria of AWGS,the elderly were also divided into sarcopenia group and non-sarcopenia group,then comparing the difference aspects between groups.We comprehensively measured all the three components(muscle mass,muscle strength,and physical performance)of sarcopenia and compared the three components among Chinese elderly with different BMD.Furthermore,we determined if sarcopenia as defined by the new criteria of AWGS is more prevalent among osteopenia/osteoporosis Chinese elderly than those with normal BMD and estimated the risk of developing sarcopenia among Chinese elderly with osteopenia/osteoporosis.Results:(1)There were significantly defference in body weight,body mass index,appendicular skeletal muscle index,grip strength,gait speed,fat mass,fat free mass,muscle mass,appendicular skeletal muscle mass and trunk muscle mass among different BMD groups.(2)In the study,28 elderly people were diagnosed of sarcopenia.The percentage of sarcopenia in 191 elderly people was about 14.66%,the prevalence of sarcopenia was higher in osteopenia/osteoporosis groups than in normal group(29.10%vs12.20%vs3.70%,χ2=14.74,P<0.05).(3)Age were higher in SAC group[(72.00±9.84)vs.(67.98±7.12),P<0.05] than that in Non-SAC group;Weight was lower in SAC group[(51.87±8.92)kg vs.(64.05±10.47)kg,P<0.01]than that in Non-SAC group;Body mass index was lower in SAC group[(19.99±2.44)kg /m2 vs.(24.36±2.96)kg/m2,P<0.05]than that in Non-SAC group;Bone mass was lower in SAC group [(2.27±0.32)kg vs.(2.72±0.52)kg,P<0.05]than that in Non-SAC group;BMD of L1-4、femoral neck、Total hip were lower in SAC group [(0.89±0.19)g/cm2 vs.(0.99±0.18)g/cm2,(0.71±0.1)g/cm2 vs.(0.81±0.13)g/cm2,(0.81±0.12)g/cm2 vs.(0.89±0.14)g/cm2,P<0.01] than that in Non-SAC group;T score of L1-4、femoral neck、total hip were lower in SAC group[(-1.84±1.58)vs.(-0.9±1.58),(-1.87±0.83)vs.(-1.13±1.04),(-1.33±0.90)vs.(-0.72±1.05),P<0.05] than that in Non-SAC group;Albumin were lower in SAC group[(37.46±7.97)g/L vs.(41.08±6.04)g/L,P<0.05]than that in Non-SAC group.Difference of cholesterol、triglyceride and fasting plasma glucose had no statistical meanings(P>0.05).(4)Binary logistic regression analyses showed that the odds ratio(OR)of osteopenia and/or osteoporosis for sarcopenia was significantly [OR=3.140,95%CI(1.667~5.916),P=0.0004].It was substantially attenuated but remained marginally significant when adjusted for age,sex,DM,cholesterol,fasting blood glucose,and albumin [OR=2.789,95%CI(1.341~5.799),P=0.006].This association was modified by body mass index [OR=1.643,95%CI(0.665~4.057),P=0.2818].Conclusion(1)Osteopenia/osteoporosis groups had lower musle mass and higher prevalence of SAC compared to that in normal group.Bone mineral density may have a certain effect on the occurrence of SAC.(2)Osteopenia/osteoporosis may independently increased the risk of prevelance of SAC,the elderly patients who were clinically older with osteopenia/osteoporosis should be screened、prevented and intervented of sarcopenia in time.
