Using Four Kinds Of Imaging Methods To Compare The Diagnostic Value Of Liver Fatty Change For The Patients With CHB And FLD

Objective: To discuss the diagnostic value of liver fatty change for the patients who have been infected by hepatitis B virus and fatty liver disease using the four kinds of commonly used imaging methods(ultrasound,FibroScan,CT,MRI).Methods: There were 109 cases that been diagnosed as chronic hepatitis B patients by hepatitis B etiology examination,and also been diagnosed as fatty liver disease by imaging examination(ultrasound,FibroScan,CT,MRI)in November 2016 to January 2018 in Yanan University Affiliated Hospital hospitalization.First of all,we should collect the result of imaging examination,and collecting their general situation and the related clinical information reversely.All patients were performed liver biopsy pathologic examination and collect the results.To increase the sample size and were collected from January 2015 to October 2015 in our hospital hospitalization and conform to the requirements of the data of 8 cases of patients,a total of 117 cases.For liver biopsy pathology test results as the gold standard comparison,using ROC curve to assessment the value of using ultrasound,FibroScan,CT and MRI to diagnostic the liver fat content in patients with hepatitis B viral hepatitis with fatty liver disease.Results: Using Ultrasound to diagnostic the fatty liver,the AUROC is 0.668,sensitivity is 0.664,a specific degree is 0.673.The AUROC is 0.783,the sensitivity is 0.727,specific degree is 0.701 in FibroScan;Using FibroScan to diagnostic liver hardness,the AUROC is 0.665,sensitivity is 0.667,specific degree is 0.660.The AUROC is 0.542,sensitivity is 0.405,specific degree is 0.684 in CT;The AUROC is 0.879,when took the best cutoff score of LFC,the sensitivity can reach 0.750,the specific degree of 0.943 in MRI.Conclusion:1.FibroScan has a higher degree of accuracy,sensitivity and specialty of diagnostic the fatty liver in patients with CHB with FLD.Due to the convenient,cheap,FibroScan may be can alternative ultrasound as a method of screening for fatty liver.However,same as ultrasound,FibroScan cannot accurately measurement the liver fat content;FibroScan for diagnosis of liver stiffness an acceptable degree of accuracy,sensitivity and specific.2.MRI also has a higher degree of accuracy,sensitivity and specialty of diagnostic the fatty liver in patients with CHB with FLD,and MRI can accurately quantify the liver fat content.3.Ultrasound has a slightly lower degree of accuracy,sensitivity and specialty of diagnostic the fatty liver in patients with CHB with FLD,if there was no FibroScan,ultrasound is still a good screening method.4.CT has a general degree of accuracy,sensitivity and specific of diagnostic the fatty liver in patients with CHB with FLD.
