Background:Chronic rhinosinusitis(CRS)is a localized chronic inflammatory disease with a reported global prevalence of about 5-12%,while our country is slightly lower than 2-8%.The treatment is mainly medicine and endoscopic surgery.Currently,CRS clinically divided into two subtypes,is based on the presence or absence of nasal polyps,each with typical characteristics of tissue remodeling and inflammation.Among them,Chronic rhinosinusitis with nasal polyps(CRSwNP)has high clinical symptom scores and more nasal symptoms,with a high recurrence rate,which is one of the refractory disease of otolaryngology and head and neck surgery.However,the pathogenesis of CRSwNP is not fully understood and remains to be further studied.Interleukin-35(IL-35)has the typical structural features of IL-12 family and is the newly identified family member,which plays an important regulatory role in T cell activation,proliferation and cytokine release.IL-35,as a novel inhibitory cytokine,has significant changes in many diseases and involves in the development of disease such as rheumatoid arthritis,viral hepatitis B,asthma and allergic rhinitis.In addition,the decreased expression of IL-35 has been shown to be associated with the development of respiratory inflammation.However,CRSwNP is a chronic inflammatory disease of nasal and sinus mucosa.Therefore,we used Real-time PCR,ELISA and FCM to analyze the expression of IL-35 in patients with CRSwNP.To provide a reliable theoretical basis for the analysis of the role of IL-35 in the pathogenesis of CRSwNP.Objective:This study was to investigate the expression of IL-35 in patients with CRSwNP,and to supplement the pathogenesis of CRSwNP.Methods:The patients with CRSwNP who hospitalized in our department from May 2016 to April 2017 were randomly selected,and have been performed nasal endoscopy and sinus CT examination.The clinical symptoms and auxiliary examination results were evaluated in detail,and intraoperative taken the nasal polyp tissue and peripheral venous blood.The expression of IL-35 mRNA(EBIB mRNA and IL-12A mRNA)was detected by Real-time PCR in nasal polyp tissues,while the cytokines of IL-35,IL-10 and IL-17 were detected by ELISA.The Th17 and Treg cells were detected by FCM in peripheral venous blood.Results:A total of 51patients with CRSwNP were collected for intraoperative nasal polyp tissue and peripheral venous blood.Among them,the VAS score was 5.5 ± 1.7,the nasal endoscopy score was 6.4 ± 2.0,the sinonasal CT score was 11.7 ±3.8.Our results of Real-time PCR analysis showed that IL-35 mRNA(EBIB mRNA and IL-12A mRNA)expression was significantly decreased in CRSwNP compared with normal nasal mucosa(P<0.05).Our results of ELISA analysis showed that the cytokines of IL-35 expression were significantly decreased in CRSwNP(148.5±46.9 pg/ml)compared with normal nasal mucosa(202.5±41.7 pg/ml)(P<0.05).The cytokines of IL-10 were significantly increased in CRSwNP(97.3±22.8 pg/ml)compared with normal nasal mucosa(68.6±11.0 pg/ml)(P<0.05).The cytokines of IL-17 were significantly increased in CRSwNP(180.8±37.1 pg/ml)compared with normal nasal mucosa(133.4±19.8 pg/ml)(P<0.05).The Th17 cells were significantly increased in CRSwNP(0.89±0.50%)compared with control group(0.54±0.21%)(P<0.05).The Treg cells were significantly decreased in CRSwNP(4.74±2.06%)compared with control group(6.10±1.20%)(P<0.05).The Th17/Treg was significantly decreased in CRSwNP(0.20±0.12)compared with control group(0.09±0.03)(P<0.05).Conclusions:These data suggest that IL-35 may play an important role in the pathogenesis of CRSwNP.
