Objective:It is still controversial whether serum uric acid(SUA)is an independent risk factor for atherosclerosis(AS),and the process of uric acid and atherosclerosis are not the same in the middle-aged and elderly populations.Therefore,there may be differences among different age groups,and the association should be analyzed separately.At present,there is little research on the analysis of the association between the two in the general elderly population.Since carotid intima-media thickness(CIMT)is an early measure of arteriosclerosis,the purpose of this study was to investigate the relationship between serum uric acid and carotid intima-media thickness in general elderly population(age ? 60 years old).Methods:The study was a cross-sectional study.The study population came from the participants in the "Tianjin Chronic Low-Grade Systemic Inflammation and Health(TCLSIHealth)cohort study".In the general elderly population(? 60 years old)recruited in the health management center of the General Hospital of Tianjin Medical University in 2016.Fasting blood and carotid ultrasound were performed to obtain serum uric acid and carotid intima-media thickness.In addition,the study population completed a detailed survey questionna ire of family history and personal habits of smoking and drinking,and finally included a total of 2,788 research population.The subjects were divided into non-hyperuricemia(NHU)group and hyperuricemia(HU)group according to the serum uric acid concentration level,and non-hyperuricemia subjects were divided into groups 1 to 4 according to the low to high uric acid level;the subject of hyperuricemia was treated as group 5.In this study,we use variance analysis and Logistic regression analysis to compare the basic demographic characteristics of the population by gender,and use multiple logistic regression analysis to further analyze the relationship between serum uric acid and carotid intima-media thickness in the total population and male and female population.All statistical analyses were performed using SAS 9.3 software.Results:The overall prevalence of CIMT thickening was 53.37%(66.62% for men and 36.77% for women).In total population between the normal CIMT group and thickening group or among the different levels of SUA,body mass index,waist circumference,the prevalence of metabolic syndrome,triglycerides,total cholesterol,low-density lipoprotein cholesterol,high-density lipoprotein cholesterol,systolic blood pressure,diastolic blood pressure,and fasting blood glucose were statistically significant.After adjustment for potential confounding factors,using group 3 as the reference group,odds ratios(OR)[95% confidence interval(CI)] for the CIMT thickening of 1 to 5 SUA levels in the general population was 1.17(0.91,1.50).1.27(0.99,1.63),1.00,1.10(0.86,1.41),and 1.46(1.12,1.91),whereas the odds ratio(95% CI)for CIMT thickening from low to high SUA levels in the male population was 1.57.(1.10,2.27),1.24(0.88,1.76),1.00,1.25(0.88,1.78),1.61(1.09,2.38);the ratio of CIMT thickening from low to high SUA levels in the female population(95% CI)is 0.90(0.62,1.29),1.34(0.94,1.92),1.00,0.94(0.65,1.35),1.30(0.89,1.91).Only the hyperuricemia group had a higher prevalence of CIMT thickening in the general population.In the general elderly male population,a moderately elevated serum uric acid level was independently associated with a lower prevalence of CIMT thickening.Low serum uric acid levels were asso ciated with increased prevalence of CIMT thickening,and the relationship between the two was non-linear,similar to the U-shaped association,while the association was not statistically significant between the two in general elderly female.Conclusion: This study found a positive correlation between hyperuricemia and the prevalence of CIMT thickening in the general elderly population.It was found for the first time that the relationship between SUA levels and CIMT subjects was U-shaped in general elderly male but not in female.
