The Relevance Between18F-FDG PET/CT Scans And Laboratory Data And The Prognostic Value Of 18F-FDG PET/CT In Patients With Newly Diagnosed Multiple Myeloma

Objective:Multiple myeloma(MM)is the second most common hematological malignancies.It can affect many organs,causing the symptom called “CRAB”,namely hypercalcemia,renal impairment,anemia or bone lesions.Bone lesion is the most common symptom,which badly impact patients’ quality of life.Therefore,the timely and accurate detection of bone lesions is necessary.The imaging methods of detecting bone lesions include X-ray,CT,MRI and PET/CT.This artical is aimed to explore the relevance between PET/CT and laboratory investigations of MM and its prognostic value.Methods:We retrospectively analyzed 123 patients in TianJin Medical University Cancer Institute and Hospital who had PET/CT images at diagnosis from September 2008 to December 2017,including their clinical features,survival time,PET/CT imagings and laboratory investigations.We used SPSS 22.0 to perform statistical analysis.The relevances between PET/CT imagings and laboratory investigations were performed by correlation analysis.Kaplan-Meier analysis and log-rank test were uesd to perform univariate survival analysis.And statistical significance was considered to be P<0.05.Results:Of 123 patients,there were 67 patients(54.5%)who had lytic bone lesions everywhere of the bodies.41 patients(33.3%)had pathological fractures,which were either rib pathologic fractures or spinal compression fractures.The levels of hemoglobin in patients who had lytic bone lesions were significantly lower than who didn’t(76.1% to 57.1%,P=0.025).The levels of creatinine in patients who had lytic bone lesions were significantly higher than who didn’t(43.3% to 21.8%,P=0.024).But lytic bone lesions had no correlations to the level of albumin,globulin,LDH,β2-MG,blood calcium,serium inorganic phosphorus,CRP and abnormal cytogenetics(P>0.05).The pathologic fractures showed no correlations to any of the level of hemoglobin,albumin,globulin,LDH,β2-MG,serum creatinine,blood calcium,serium inorganic phosphorus,CRP and abnormal cytogenetics(P>0.05).Lytic bone lesions and pathological fractures both showed no correlations to PFS and OS(P>0.05).EMD were found in 43 patients(35.0%),the levels ofβ 2-MG were significantly higher in patients who had EMD(P=0.045),other laboratory investigations had no significantly relevance to EMD(P>0.05);Kaplan-Meier analysis showed that EMD was a poor prognostic factor for PFS and OS(P=0.043;P=0.049),and multivariate Cox regression analysis showed that EMD was an independent prognistic factor.When SUVmax≥5.7 on PET/CT,the patients had shorter OS than whose SUVmax < 5.7(P=0.032).But SUVmax was not an independent risk factor and had no difference to PFS(P>0.05).Conclusion:In this article,we found that the levels of hemoglobin in patients who had lytic bone lesions were significantly lower than who didn’t.The levels of creatinine in patients who had lytic bone lesions were significantly higher than who didn’t.Lytic bone lesions were relative to laboratory investigations,which suggests they might be indirect prognostic factors.EMD was an independent poor prognostic factor which was in accordance with published articles.SUVmax on PET/CT was not an independent poor prognostic factor for OS but not PFS,which was not exactly in accordance with published articles.The possible reasons might go to the small sample and information bias caused by this retrospective study.Also,the therapy has changed from 2008 to 2017 and the patients’ ages included all ranges,which all impacted the results.What’s more,this argument is still controversial.Therefore,the relevance between SUVmax on PET/CT and survival time needs prospective studies to further researches.
