Objective:To learn about the influencing factors of HBe Ag status and virus level in pregnant women with hepatitis B from the combined perspective of social and biological factors.To understand the changing situation of HBeAg and HBV DNA level better and comprehensively,and then provide suggestions for blocking mother-to-child transmission of hepatitis B.Methods:Adopted the method of cluster sampling,the medical record information of 1565 pregnant women who went through with prenatal examination and delivered at Wuhan Women and Children Medical and Healthcare Center,Wuhan,China from June 2012 to August 2017 was collected.Social demographic characteristics,past medical history,gestation history and situation of pregnancy were investigated.Descriptive analysis,Chi square test and unconditional binary logistic regression analysis were adopted to analyze data.Results:1.18.40%(288/1565)of pregnant women enrolled were HBeAg-positive;65.52%(918/1401)were HBV DNA-negative(?10~3 copies/m L);34.48%(483/1401)were HBV DNA-positive.196 women were with high viral load(?10~6 copies/mL),accounting for 40.58%of the DNA-positive ones and 13.99%of all subjects.2.Univariate analysis results showed that age,operation history,HBV DNA status(viral load)of last trimester,height and ALT level were associated with HBeAg status.Multivariate analysis results showed that a greater number of women in the?24 years group and 25-29 years group were found to be HBeAg-positive(OR 2.83,95%CI:1.18~6.79,P<0.05;and OR 2.38,95%CI,1.22~4.65,P<0.05 respectively)when compared with the?35 years group.HBV DNA-positive group had a higher prevalence of HBeAg-positivity(OR 13.98,95%CI:9.15~21.37,P<0.05),and high viral load group also had a higher prevalence of HBeAg-positivity(OR 19.32,95%CI:10.59~35.26,P<0.05).3.HBeAg status of last trimester and ALT level(>40U/L)were significantly associated with HBV DNA positivity(OR 13.97,95%CI:9.57~20.40,P<0.05;and OR 1.57,95%CI:1.08~2.27,P<0.05 respectively).4.Univariate analysis results showed that age,times of prenatal examination and HBeAg status of late trimester were associated with HBV viral load.Multivariate analysis results showed that the viral load of HBeAg-positive women was significantly higher than HBeAg-negative ones(OR 18.10,95%CI:10.23~32.00,P<0.05).Conclusions:Age was significantly associated with HBeAg status among pregnant women under 30 years old.HBeAg-positive status was associated with HBV DNA positivity and high viral load.ALT level of last trimester was positively correlated with HBV DNA positivity.
