Objective:To investigate the value of 99mTc-MDP quantitative bone SPECT/CT imaging with SUVmax for the evaluation of the regularity of condylar growth activity with age in the normal occlusion individuals,and the condylar growth activity in the patients with facial asymmetry.Materials and methods:One hundred normal occlusion individuals?58 female and 42 male,aged 13-71 years,mean age 39.9±15.3 years?and 13 patients with facial asymmetry?8 female and 5 male,aged17-27 years,mean age 20.5±3.4 years?were underwent maxillofacial SPECT/CT imaging after injection 99mTc-MDP.SIEMENS Symbia Intevo16 SPECT/CT was used for data acquisition,XSPECT/CT Quant reconstruction and quantitative analysis.The volume of interest?VOI?of bilateral condyles was delineated and the SUVmax?maximum standardized uptake value?of each condylar was measured.The difference of condylar SUVmax in the gender,age and bilateral condyle between normal and abnormal individuals were analyzed respectively.Different groups were divided according to the age and were analyzed the trend of changes in the condylar SUVmax with age in normal individuals.Statistical analysis was performed with Chi square test.Difference between groups was statistically compared using the Non-parametric test or one-way analysis of variance.The optimal cut-off value of condylar SUVmax for recognizing abnormal condylar activity in the age of 17-27 years old with the ROC curve analysis was evaluated.The difference in the SUVmax between the bilateral condyles was calculated,and the optimal cut-off value was assessed as well.Results:1.?1?There was no statistical significant difference in condylar SUVmax between male?3.91±1.43?and female?3.94±1.16??Z=-0.662,P=0.508?,left?3.84±1.20?and right?4.02±1.35?condyles?Z=-0.694,P=0.488?in normal people.?2?The condylar SUVmax of the 10-19 years old group was 5.9±1.4,and the condylar SUVmax of the 20-29 years old group was 4.5±1.2.The mean condylar SUVmax after the age of 30 did not exceed 4.0.The difference in the SUVmax between the bilateral condyles in the normal population was 7.3%±6.0%.?3?The SUVmax was rising along with the increasing of age before 18 years old,then gradually decreasing until balance around 27 years old.2.?1?The abnormal lateral condylar SUVmax?6.30±1.03?was significantly higher than the contralateral side?4.86±1.18??Z=-2.616,P=0.009?in patients with facial asymmetry.Theabnormal side condylar was more plump than the contralateral side.?2?The SUVmax of the abnormal condyles in patients with facial asymmetry?6.30±1.03?was much higher than that of the normal condyles?4.90±1.37??Z=-3.373,P=0.001?.There was no statistical significant difference in condylar SUVmax between the other condyles in patients with facial asymmetry?4.86±1.18?and the normal condyles?4.90±1.37??Z=-0.061,P=0.951?.The difference in SUVmax between the bilateral condyle in patients with facial asymmetry was 13.6%±8.2%,and it was 7.3%±7.2%in the same age group of 32 normal people?Z=-2.467,P=0.014?.?3?The cut-off value of the difference in SUVmax between the bilateral condyles to distinguishabnormal condyles was 9.15%.The cut-off value of SUVmax to identify abnormal active condyles was 5.575 for the age of 17-27 years old.Conclusions:The SUVmax of condyle in 99mTc-MDP quantitative bone SPECT/CT imaging was used to assess condylar growth activity.There was no significant difference in condylar growth activity between gender and bilateral condyles in normal people.The variation of SUVmax along with age demonstrated the tendency of condylar growth activity would be balanced around 27 years old.The cut-off value of the difference in SUVmax between the bilateral condyles can help to identify abnormal condyles,and the SUVmax may be used clinically for assessing the appropriate time of the surgical treatment for the patients with condylar hyperplasia.
