ObjectiveTo retrospectively analyze the clinical characteristics of the elderly women inpatients with chronic heart failure.MethodsThe elderly women inpatients with heart failure(NHYA classification II-IV and age?60 years)were enrolled in our hospital from 2016.04.01 to 2017.03.31.HF met the criteria of the criteria of 2013 ACCF/AHA for heart failure management guidelines.According to gender,the patients were divided into the elderly women HF and the elderly men HF group.The elderly women HF group was further divided into the elderly women with ejection fraction reduced heart failure subgroup(HFrEF group,EF?40%)and the elderly women with ejection fraction preserved heart failure subgroup(HFpEF group,EF>40%).The characteristics of the elderly women HF were analyzed and the differences between HFrEF and HFpEF in the elderly women were compared.Results1.There were 278 the elderly patients with HF,including 168 cases women and with an average age of 75.7 ± 7.6 years in the study.There were 136 and 32 cases in HFpEF group(81%)and HFrEF group(19%)respectively.2.Hypertension,coronary heart disease and valvular heart disease were the major causes of heart failure in the elderly women.WBC,HB,RDW,URIC,CREA,QRS,LVED,LVES and smoking in the elderly women HF were lower than those in the elderly men with HF(P<0.05).Both levels of TG and LDL-C were higher than those of the elderly men with HF(P<0.05),multivariate logistic regression analysis showed that HB,RDW,CREA,QRS,LVED,LVES between the two groups remained statisticant differences significant(P<0.05),but there were no significant difference in blood pressure,BMI,NT-proBNP,comorbidities,drug use,the average length of hospital stay and hospitalization costs between the elderly men and women with HF(P>0.05).3.The age,BMI,systolic blood pressure,atrial fibrillation and anemia in the HFpEF group were higher than those in the HFrEF group(P<0.05).The main causes of HFpEF group were hypertension and coronary heart disease.But the comorbidities of diabetes,stroke,pneumonia,COPD,renal insufficiency and so on were similar to the HFrEF group(P>0.05).WBC,NT-proBNP,HB,URIC,CREA and HR in the HFpEF group were lower than those in the HFrEF group(P<0.05).The levels of TG,CHOL,LDL-C,HDL-C and QT interval of ECG in the HFpEF group were higher than those in the HFrEF group(P<0.05).multivariate linear regression analysis showed that URIC,HR,QRS,QT,LVED,LVES between the two groups remained statisticant differences significant(P<0.05).The use of CCB(47.8%)was higher but the use of ACEI/ARB,diuretics,beta-blockers,digitalis and lipid-lowering drugs were lower in the HFpEF group than in the HFrEF group(P<0.05).There was no significant difference in the use of aldosterone receptor antagonist,warfarin and antiplatelet drugs between the HFpEF group and the HFrEF group(P>0.05).Conclusions1.HFpEF accounts for the majority of heart failure in the elderly women.Furthermore,hypertension,coronary heart disease,and valvular disease are the major causes of HF in the elderly women.2.The age,BMI,systolic blood pressure and both morbidities of atrial fibrillation and anemia of HFpEF in the elderly women are higher than those of HFrEF in the elderly women.
