Objective:This study was purposed to make judgement about the impact of EBV infection on lymphoma prognosis by an retrospective analysis method,which investigate the EBV infection status of lymphoma patients with different pathological types and that of pragnosis relativity.Materials and Methods: The retrospective cohort methods was used to analyze the information of lymphoma patients from January 2014 to January 2018 in our Hospital which included pathological result,clinical and laboratory characteristics,treatments and prognosis.The positive rate of EBV were studied to evaluate the relationship between EBV infection and lymphoma patients with different pathological types.The different clinicopathologic features of lymphoma with or without EBV infection were analyzed by Chi square test and Fish exact probability method.Kaplan-Meier and Cox survival analysis was also applied to investigate the EBV effect on overall survival of DLBCL patients with EBV infection.SPSS 24.0 software was applied for statistical treatment.Results1.This study included 106 cases of lymphoma,67 males and 39 females,with an average age of 53 years(6~84 years).In this study,106 cases of lymphoma were included,Among them 5 in 10 patients with HL were proved to be positive(50%),and 30 in 96 patients with NHL were proved to be positive(31.25%),in which the B-NHL patients were 70 cases,14 cases(20%)were positive,the T-NHL patients were 16 cases,8 cases(50%)were positive,the NK/T-NHL patients were 10 cases,9 cases(90%)were positive.Comparing the EBV infection status of lymphoma patients with different pathological types,we come to a conclusion that there was a difference between the pathological types of lymphoma and EBV infection rate,the difference was statistically significant(P < 0.05).Among the B-NHL,the positive rate of EBV infection in DLBCL was 30.43%,the rest of B-NHL proved to be no EBV infection,so the positive rate of EBV infection between DLBCL and other B-NHL were different(P < 0.05),and the positive rate of EBV infection in DLBCL was obviously higher than that in other B-NHL(30.43% vs 30.43%).Among the T-NHL,the positive rate of EBV infection in AITL was 100%,and the positive rate of EBV infection in other T-NHL was 27.27%,so the positive rate of EBV infection between AITL and other T-NHL were different(P < 0.05),and the positive rate of EBV infection in AITL was obviously higher than that in other T-NHL(100% vs27.27%).The positive rate of EBV infection in NK/ T-NHL was 90%,and there was 1 case that could be wrong in the 6 cases which was proved negative by IHC.2.The clinical factors were analyzed respectively to figure out the correlative ones in HL and NHL.We analyzed the prognostic factors of HL and found that positive and negative groups which divided by EBV infection status had different radio in age(> 45),the proportion of older patients in positive group was obviously higher than that in negative group(P < 0.05).In the analysis of NHL prognostic factors,the proportion of patients with EBV infection in age(>60 years and ≤60 years old)was different,and the difference was statistically significant(P < 0.05),and the proportion of patients with EBV infection(> 60 years)was significantly higher than patients without EBV infection.In the comparison of different pathological types(B,T,NK/T),the proportion of NHL patients with EBV infection was different in each pathological classification,among which the proportion of B-NHL patients was significantly lower than that of patients without EBV infection,while the proportion of patients with T-NHL and NK/T-NHL was higher than that of patients without EBV infection,and the difference was statistically significant(P < 0.05).In the comparison of systemic symptoms(symptom B and A),the proportion of patients with or without EBV infection was different,and the difference was statistically significant(P < 0.05),and the proportion of NHL patients with EBV infection was significantly higher than those without EBV infection.3.Kaplan-meier method was used for survival analysis of DLBCL patients,and verified by log-rank test showed that the survival rate between positive or negative groups divided by EBV infection status was different(P<0.05),EBV infection was proved to be a risk factor of DLBCL.Multivariate prognostic analysis model showed that DLBCL patients with EBV infection were worse than without EBV infection in survival.Comclusion1.EBV infection was not the same in different pathological types of lymphomas.2.The rate of EBV infection in HL and NHL patients might increase with age.In NHL,patients with T cells and NK/T cells had higher EBV infection rates.Meanwhile,compared with group A,NHL patients with group B symptoms had higher EBV infection rate.3.The survival of DLBCL patients with EBV infection was worse than those without EBV infection.
