| Background:Patients with mechanical ventilation have the characteristics of severe illness and rapid change.Timely assessment and close observation of patients’oxygenation status plays an important role.Oxygenation index is the most common monitoring index,but because of its invasive,discontinuous,lack of timeliness and other defects,it limits the use of this index in the process of nursing evaluation.The oxygen saturation related diffusion index as the index dispersion of noninvasive oxygenation of patients with mechanical ventilation function and disease development,make up for the shortcomings of oxygenation index.The consistency between the index of oxygen saturation related diffusion index and oxygenation index is not clear.The critical value of oxygen saturation related diffusion index for assessing the severity of oxygenation in mechanically ventilated patients remains to be further explored.Objective:To verify the consistency between the oxygen saturation related diffusion index and the oxygenation index in determining oxygenation status.To establish the critical value of the oxygen saturation related diffusion index for assessing the severity of oxygenation in mechanically ventilated patients.Methods:Non-experimental descriptive study was used in this study.From October 2016 to March 2017,patients who were hospitalized in the EICU of Peking Union Medical College Hospital and who met the inclusion criteria were included in the study.Arterial blood gas analysis,ventilator setting parameters and pulse oximetry monitoring data were collected,oxygenation index and oxygen saturation related diffusion index were calculated.Results:(1)There was a positive correlation between the oxygen saturation related diffusion index and oxygenation index(r=0.670,P<0.01),linear equation is Y(oxygenation index)=73.689+0.323X(the oxygen saturation related diffusion index),the significance test of the equation was statistically significant(P<0.05),which proved that the two indexes were consistent with the monitoring of oxygenation status.(2)The critical value between the moderate and severe groups of the oxygen saturation related diffusion index was 193(sensitivity 73.1%,specificity 71.8%),and the critical value between mild group and moderate group was 483(sensitivity 75.3%,specificity 90.0%).The oxygen saturation related diffusion index failed to determine the critical value in mild group and normal group.Conclusions:The oxygen saturation related diffusion index and oxygenation index are consistent with oxygenation status monitoring in patients with mechanical ventilation,and can be used as a noninvasive indicator for monitoring oxygenation status.According to the critical value of the oxygen saturation related diffusion index,nurses can effectively identify the oxygenation status of patients with mechanical ventilation,and closely observe the condition,at the same time,reduce the damage and pain caused by the collection of arterial blood samples. |
