| Objective Object to investigate the clinical efficacy of low molecular weight heparin(LMWH)in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and its effect on the remission rate within 1 years.Methods 70 patients with AECOPD during May 2015 to October 2015 in our hospital department of respiration were randomly divided into parallel experimental design,we had low molecular weight heparin combined with conventional treatment group(observation group)and conventional treatment group(control group).The two groups were compared in patients admitted to the dyspnea score,blood gas analysis call before and after the treatment of coagulation related indicators.Pulmonary function indexes were observed after the treatment,the same as the 1 year of follow-up in the hospitalization time of remission time and changes in the rate of remission.Results There were no significant differences in age,sex,body mass index(BMI),course of disease,non-invasive mechanical ventilation ratio,m MRC score,PaO2,PaCO2 and SaO2 between the two groups before the treatment(P > 0.05).There were significant differences in m MRC score,Pa O2,Pa CO2 and Sa O2 before and after treatment in the observation group(P < 0.05).After treatment,Pa O2,Pa CO2,Sa O2 were significantly different between the two groups(P < 0.05),suggesting that low molecular weight heparin sodium combined with routine treatment can improve the symptoms of dyspnea,improving hypoxemia and hypercapnia;there was no significant difference in D-dimer,APTT,PT,TT,FIB between the observation group and the control group before treatment(P > 0.05).There were significant differences in D-dimer,APTT,PT,TT and FIB in the observation group before and after treatment(P < 0.05).There were significant differences in D-dimer,APTT,PT,TT and FIB between the observation group and the control group after treatment(P < 0.05).The second forced expiratory volume(FEV1)and the first second forced expiratory volume / forced vital capacity(FEV1 / FVC)of the observation group were better than those of the control group(P < 0.05),and the hospitalization time of the observation group was shorter than that of the control group(P < 0.001),suggesting that low molecular weight heparin sodium combined with conventional treatment can improve pulmonary function and shorter the hospital stays.The clinical remission time of the two groups was longer in the observation group than in the control group(P = 0.04),suggesting that low molecular weight heparin sodium combined with conventional treatment can prolong the interval of the second acute attack of COPD.Conclusion Low molecular weight heparin combined with conventional therapy can significantly reduce the symptoms of dyspnea,improve hypoxia and hypercapnia,alleviate blood coagulation,optimize lung function,shorten hospitalization time,and prolong the interval of the second acute attack of COPD. |
