Objective: To explore the curative effect and prognosis of patients with initial non-resectable liver metastasis of colorectal cancer treated with different transformation therapy.Methods:The clinical data of 179 patients with initial unresectable liver metastasis of colorectal cancer from January 2010 to December 2015 in the third Clinical Medical College of Xinjiang Medical University were retrospectively analyzed.To explore the efficacy and safety of transforming therapy in patients with initial unresectable liver metastasis of colorectal cancer discussed by MDT.KaplanMeier univariate and Coxmultivariate risk model were used to analyze the prognos tic factors of patients with primary unresectable liver metastasis.Results: There were 46 cases of liver metastasis resectable after transformation therapy,including 15 cases of simple chemotherapy,18 cases of chemotherapy combined with targeted th erapy,and 13 cases of chemotherapy combined with local treatment.The adverse r eactions were mainly grade 1 to 2,and 26.0% of the patients had complications during the period of transformation therapy.The univariate results of 179 patients showed that "time of occurrence of liver metastasis","whether or not liver metastasis was resected","size of liver metastasis" and "transformation therapy" had an effect on 3-year 0S of patients(all P < 0.05).COX multivariate analysis showed that "liver metastasis resection" and "treatment method" were independent risk factors for prognosis.Conclusion:The survival time of patients with initial unresectable liver metastasis of colorectal cancer can be improved by resection of liver metastasis after transformation therapy.Chemotherapy combined with molecular targeting drugs can improve the prognosis of patients.At the same time,conversion therapy does not increase the risk of perioperative period,most of the patients can tolerate adverse reactions during the transformation therapy.Therefore,the transformation therapy has certain safety,for the initial unresectable colorectal cancer liver metastasis patients,we should actively carry on the transformation treatment.
