Objective Individualized exercise can effectively control blood pressure, but the mechanism is unclear. The waveforms of brachial artery pulse waves in resting and their changes before and after exercise in chronic hypertensive patients and normal persons were calculated and compared, and the similarities and differences were compared in order to find evidence for exercise control of hypertension and explore possible mechanisms. Methods Eleven cases of chronic diseases patients who recognited holistic program management were recruited, eight cases of normal subjects as control. The radial pulse wave was recorded before and after individualized exercise.Preliminary mapping analysis:the original data was remapped, and the frequency of notch was calculated, and the waveform preliminarily classify according to whether the notch appearance occurrence(the transition between the two waves),and the notch appearance rate before and after exercise was compared.Resting data analysis:seven cases of hypertensive and seven cases of normal subjects(excluding 1 suspected hypertensive person)were selected and pulse waveform was further classified according to the characteristics of its rising and falling, the differences of waveform type between the two groups were compared. The y-magnitude include starting point(B),the first wave crest(P1),crest of the two wave(PL),the second wave crest(P2)and the ending point(E,the next wave starting point)and x-time of the pulse wave was analyzed and calculated, and the linear slope between the points, the total slope of the descending branch and the relative and absolute ratio to P1.The resting pulse wave of hypertension and normal subjects were analyzed from above parameters.Pre-and post-exercise data analysis:Parameters were calculated and analyzed like resting pulse wave, and then pre-and post-exercise changes and similarities and differences between groups was compared.Calculate the difference of parameters post-and pre-exercise,comparing similarities and differences of them.Notch occurrence rate was analyzed by chi-square test. The pulse wave parameters between groups was compared by independent sample t-test,and repeated measures analysis of variance was used to compared parameters before and after exercise. Results Before exercise, only 6 cases of 11 patients had a notch;while all normal subjects had a notch, and the notch occurrence rate was lower than normal(P<0.05).The the notch occurrence rate in patients after exercise increased, but there was no statistical difference(p>0.05).At rest, most of pulse wave were type A(85.7%)in the normal subjects(type A:the second wave was obvious);most of pulse wave type C(42.9%)in hypertensive(type C:with no notch),B of hypertensive was lower than,P1 was higher than that of Normal subjects(P<0.05).After exercise, patients and normal individuals showed a deepening of the notch amplitude, the patient’s notch platform increased, patient B increased,P1 decreased, while normal subjects B decreased,P1 increased(P<0.05).Conclusion The patients pulse wave of radial artery is different from normal subjects at resting and changes significantly after exercise. This suggests that it can provide reference evidence for the diagnosis and the mechanism and efficacy of individualized precise exercise in managing and controlling blood pressure.
