| Objective:To compare the diagnostic value of endoscopic ultrasonography and MRI in T staging of rectal cancer.Methods:820 patients were collected from January 2012 to January 2017 at the cancer hospital Chinese academy medical sceiences by endoscopic ultrasonography and MRI examination. The cases of ultrasound and MRI images were collected and experienced doctors staged these and compared these with the postoperative pathology and discussed the diagnosis value of endoscopic ultrasonography and MRI in the preoperative T staging of rectal cancer.Result:A total of 820 patients were enrolled in this study, including 464 males and 356 females, with an average age of 54±9.6.Among them, there were 154 cases of EUS stage in T1 stage,of which 26 cases were over staging;283 cases were in T2 stage,of which 19 cases were over staging,10 cases were inadequately staging;287 cases were in T3 stage,9 cases were over staging,40 cases were inadequately staging;96 cases were in T4 stage,30 cases were insufficient. In MRI stage results,156 cases were in T1 stage,of which 34 cases were over staging,272 cases were in T2 stage,of which 17 cases were over staging,15 cases were insufficiently staging,290 cases were in T3 stage,of which 5 case were over staging,34 cases were insufficiently staging;T4 stage 73 cases,29 cases were insufficient staging. The accuracy rate of EUS for T staging of rectal cancer was 83.7%,T1,T2,T3 and T489.5%84.1%、80.7%、82.5%.Sensitivity and specificity were 89.5%、84.1%、807%、825%,96.0%、94.4%、90.7%、95.9%.The accuracy rate of MRI for T staging of rectal cancer was 83.6%,T1,T2,T3 and T4 were 85.3%、79.5%、85.1%、91.2%.Sensitivity and specificity were 85.3%、79.5%85.1%、91.2%、95.0%、93.8%、96%、961%.EUS combined with MRI for the accuracy of T staging for rectal cancer was 89.8%,T1,T2,T3 and T4 were 93.7%、88.0%、88.I%、95.0%.Conclusion:In the evaluation of rectal cancer T staging,EUS was more accurate than MRI in T1 and T2 staging, but MRI was superior in T3 and T4 stages.Combining with EUS and MRI diagnosis can further improve the accuracy of preoperative T staging of rectal cancer, and better guide clinical treatment. |
