Study On The Medical Security System Of Hospice Care To The Elders

With the aging of the population,the change of human disease spectrum and the improvement of the consciousness of life quality,the hospice care is getting more and more attention.Hospice care is an ideal model for the development of medical services for the elderly,which can lead to a more dignified death and save medical costs.In recent years,China has paid more attention to hospice care and launched a series of policies to promote the development of hospice care.At the institutional level,hospice care services are explored to be included in the basic medical system and other supplementary medical insurance.Recently,the domestic research has just carried on the preliminary exploration to the hospice care from medical perspective.The in-depth research from the perspective of social security is rare and lack of research on the medical insurance system of hospice care.Through theoretical and empirical research,this study will take Shanghai as an example to explore the rationality of the current hospice care medical insurance system and its impact on both doctors and patients,specifically analyze the problems and causes in the operation process.And it will also try to put forward suggestions for the improvement of the medical insurance system.This study mainly use the method of literature analysis,participatory observation and case interview to collect information,and use the method of causal analysis and comparative analysis to analyze materials.The study found that Shanghai,as the forerunner of hospice care,is at the leading level in terms of hospice care coverage and service delivery,and has achieved certain effect in the construction of hospice care insurance system.Hospice care insurance has improved the utilization of hospice care services.Medical insurance reimbursement relieves the financial pressure of patients and their families,and they are satisfied with the current hospice care medical insurance.However,there are still many problems.The limited coverage,unreasonable fee-for-service payment method,undefined service standards and expense system,and imperfect supporting facilities of the system will affect the sustainable development of hospice services.In this regard,this study mainly draws lessons from the advanced experience in the construction of hospice medical insurance system in the United States and Taiwan,and puts forward corresponding Suggestions for system improvement.First of all,we should expand the security project of hospice medical insurance,improve the payment method of hospice medical insurance and define the service standards and cost system to explore the hospice medical insurance system suitable for China.At the same time,relevant supporting mechanisms of the hospice medical insurance system should be improved,such as establishing a government-led multiple fund-raising mechanism,strengthening the construction of hospice personnel,improving the supervision and management mechanism,and integrating with long-term care insurance.
